摘  要:目的:探讨OSAHS患者清醒及诱导睡眠状态多层螺旋CT扫描方法,评价其临床价值。材料和方法:对43例OSAHS患者鼻咽顶部至舌骨下缘之间的上气道,进行清醒和诱导睡眠状态螺旋CT扫描,层厚2mm,螺距5.5。扫描完后应用图像工作站进行MPR三维重建,测量分析清醒和睡眠呼吸暂停时上气道最狭窄处截面积的动态改变。患者均行手术治疗并评价其术后疗效。结果:CT扫描显示患者睡眠呼吸暂停时上气道最狭窄处截面积比清醒时明显变小或完全阻塞。其中上气道最狭窄处完全阻塞20例,多部位狭窄20例。手术后住院期间打鼾消失4例;打鼾减轻35例;无明显变化4例。结论:诱导睡眠期CT扫描方法简便易行,能直观地反映上呼吸道的梗阻部位;对治疗计划制定和手术定位是有指导作用。Purpose: To investigate methods and clinical value of CT scan of obstructive sleep apneahyponea syndrome (OSAHS) at time of awaken and induced Sleep. Methods and Methods: Forty-there patients with OSAHS were examined with multi-slice spiral C T at time of awaken and asleep state, the scanning range extended from superior wall of nasopharynx to the inferior border of hyoid bone, with thickness set at 2mm, pitches at 5.5. The imaging data of all patients were reconstructed with multi-plan reconstruction(MPR) on worksta- tion, measuring cross-section areas of upper airway at time of awaken and asleep state. All cases were confirmed by operation and postopera- tive effects were evaluated. Results: Cross-section areas, of upper respiratony airway of obstructive sleep apnea hyponea syndrome at time of asleep state was significantly less than those of at time of awaken state or obstructed completely, among them, 20cases with complete obstruc- tion upper respiratony airway, 20 cases with many narrowing, of all cases, 4 cases with snore , 4 cases with snore disappearing. 4 cases with on significant change. Conclusion: CT scan at an asleep state is relatively convenient and helpful for assessing directly obstructive location of upper respiratony airway, for determining therapy plan and location.
