摘　　要：目的:总结布氏杆菌性脊椎炎流行病学特点及最佳药物治疗方案,为提高对该病的药物选择及治疗效果提供科学依据。方法:采集113例确诊患者临床流行病学资料,采用5种不同药物,分成5组不同配伍方法,进行不同疗程治疗和对比研究。结果:布氏杆菌性脊椎炎女性发病率高,平均年龄41岁,坝上发病率高于坝下,主要集中在饲养、放牧牲畜而受染,病灶分布多在腰椎,2个椎体受累最多,涉及L4的发病率最高并破坏最为明显;含强力霉素治疗组效果优于未含强力霉素组,差异有显著性(P<0.05),如增加疗程,疗效增加不明显。结论:布氏杆菌性脊椎炎临床流行病学具有特征性;以强力霉素+庆大霉素+磺胺甲基异唑为首选的抗菌素,应用两个疗程是治疗和防止本病复发最合理的方法。Objective To generalize the clinical epidemiodogical features of brucellar spondylitis and its relative therapeatical approaches to provide scientific dependencies for the improving of diagnostic level of this diease. Methods Clinical epidemiodogical materials were collected from 113 patients with brucellar spondylitis. All these patients were divided into 5 different groups according to 5 kinds of drugs adopted respectively, and then the patients were given different course of treatment. Results In the 113 patients, brucellar spondylitis morbility of female patients were higher than that of male ones, and the morbility of Bashang were higher than that of Baxia. These patients were infected mainly through browsing and breeding beasts. Lumbars were the major focus of infection. It was very common that two adjacent lumbars were involved in concurrently. L4 was the most common infection location and its demolishment was most serious. The curative effect of group treated with doxycycline was better than that of group treated without doxycycline. If the course of treatment was increased, the curative effect was not increased obviously. Conclusions There are characteristic features in clinical epidemiology of brucell spondylitis. Using antibiotics adequately and jointly by varied approaches for a long time is the major and reliable method to treat the disease and prevent the disease from recurrence.
