摘 要:目的:测定酯类局麻药丁卡因与酰胺类局麻药单用及联合肌注小鼠半数致死量(LD50),并探讨两种类型局麻药合用时的毒性相互作用形式。方法:清洁级雄性小白鼠105只,随机分为丁卡因组、利多卡因组、罗哌卡因组、布比卡因组、丁卡因-利多卡因组(丁利组)、丁卡因-罗哌卡因组(丁罗组)、丁卡因-布比卡因组(丁布组),共7组,每组15只。采用序贯法测定各组局麻药小鼠肌肉注射的LD50,求出Q值,并进行药物相似联合作用的Q检验。结果:小鼠LD50在丁卡因组为21.2mg/kg,利多卡因组为186.9mg/kg,罗哌卡因组为39.1mg/kg,布比卡因组为25.7mg/kg;丁利组为丁卡因10.3mg/kg,利多卡因82.2mg/kg;丁罗组为丁卡因12.8mg/kg,罗哌卡因19.1mg/kg;丁布组为丁卡因14.2mg/kg,布比卡因14.2mg/kg。丁利组的Q值<1,丁罗组、丁布组的Q值>1,但P值均>0.05,差异无显著性。结论:小鼠肌注局麻药全身毒性反应中,局麻药毒性丁卡因>布比卡因>罗哌卡因>利多卡因,丁卡因与酰胺类局麻药联合应用时全身毒性表现为相加作用。Objective: To detect lethal dose 50 (LD50) of single or combination of tetracaine and amide anesthetic for mice and to investigate the fashion of interaction when ccmbinating two local anesthetics. Methods: One hundred and five healthy male mice weighing (20 ± 2)g were randamly divided into 7 groups (n=15): tetracaine group, lidocaine group, ropivacaine group, bupivacaine group, tetracainelidocaine group, tetracaine-ropivacaine group and tetracaine-bupivacaine group. Single or combination of local anesthetics was intramuscularly injected into mice by up-and-down method to determine LD50 of the drugs. On the basis of these data, the Q value could be acquired, and Q-test was done to verify the combination effects of two similar drugs. Results: LD50 of tetracaine, lidocaine, ropivacaine, bupivacaine was 21.2 mg/kg, 186.9 mg/kg, 39.1 mg/kg and 25.7 mg/kg, respectively. There were 10.3 mg/kg tetracaine and 82.2 mg/kg lidocaine in LD50 of tetracaine-lidocaine group, 12.8 mg/kg tetracaine and 19.1 mg/kg ropivacaine in LD50 of tetracaine ropivacaine group, 14.2 mg/kg tetracaine and 14.2 mg/kg bupivacalne in LD50 of tetracaine-bupivacaine group. The Q-value of tetracaine-ropivacaine group, tetracaine-bupivacaine group were more than 1, and it was less than 1 in tetracaine-lidocaine group, But there was no statistically significance. Conclusion: The systemic toxic reactions of local anesthetics ranging fram high to low are tracaine, bupivacaine, ropivacaine and lidocaine when intramuscularly administrated. The interaction of tetracaine and amide anesthetic is additive.
