摘  要:目的:探讨表面活性蛋白B(SP-B)、甲状腺转录因子-1(TTF-1)及核增殖抗原Ki-67在儿童先天性肺囊性腺瘤样畸形中的表达及其意义。方法:采用EliVision法进行免疫组织化学染色,52例先天性囊性腺瘤样畸形作为病例组,9例儿童正常肺组织作为对照组,检测SP-B、TTF-1、Ki-67在肺泡、呼吸性细支气管及囊壁组织中的表达与分布。结果:SP-B与TTF-1在正常肺组织及病例组中的表达部位、强度较一致,而在先天性肺囊性腺瘤样畸形不同组织亚型中表达强度明显不同。Ki-67在正常肺组织表达为阴性,在病例组中明显增加(P<0.01),而在先天性肺囊性腺瘤样畸形不同组织类型中的表达强度差异无显著性(P>0.05)。结论:SP-B、TTF-1的表达与儿童先天性肺囊性腺瘤样畸形的组织分型密切相关,对了解先天性肺囊性腺瘤样畸形的组织发生具有重要意义;Ki-67与先天性肺囊性腺瘤样畸形的形成有关。Objective: To investigate the expression of thyroid transcription factor 1 (TTF-1), surfactant protein B (SP-B) and Ki-67 in both pediatric congenital cystic adenomatoid malformation of lung (CCAM) and normal lungs; and their's value in histologic typing and formation of CCAM. Methods: Immunohistochemical method was used to detect the expression of TTF-1, SP-B and Ki-67 in 9 normal children's lungs and 52 congenital cystic adenomatoid malformation of lung cases. Results: The expression levels and parts of SP-B and TTF-1 were uniform. At the different typing of congenital cystic adenomatoid malformation of lung, the expression of TTF-1 and SP-B was different in quantity. In the control group, Ki-67 was not detected in the nuclei of epithelial cells lining the bronchus and alveoli pulmonis. The expression levels of Ki-67 in the diseased groups were significantly higher than those in the control group (P〈0.05), but there was no significantly different among the diseased groups(P〈0.05). Conclusions: TTF- 1 and SP-B can serve as a marker for histologic typing of congenital pulmonary cyst. Ki-67 play an important role in the formation of the congenital pulmonary cysts.
