老年人肝内外胆管结石的外科治疗选择

摘　　要：目的探讨老年人肝内外胆管结石的治疗方法。方法老年肝内外胆管结石患者396例,共分为3组,其中保守治疗组82例,急诊手术组112例,择期手术组202例。结果非手术组死亡2例。急诊手术组出现术后并发症43例(38.4%),死亡2例(1.8%);择期手术组出现术后并发症33例(16.3%),死亡1例(0.5%),两组在术后并发症发生率上有显著差异(P<0.05)。结论老年人肝内外胆管结石应采用多种治疗方法与手术方法联合的个体化治疗方案,在条件允许的情况下,尽量采用择期手术。Objective To discuss the therapy choice for intra- and extra-hepatic calculus of bile duct in aged patients. Method 396 aged patients with intra- and extra-hepatic calculus of bile duct were divided into 3 groups, in which 82 received expectant treatment, 112 underwent emergency operation, and 202 underwent selective operation. Results 2 cases of the conservative group died. The complication rate of the emergency operation was 38.4% （43/112 ） , which was significantly higher than 16. 3% （33/202） of the selective operation（P 〈0, 05）. And the peri-operative mortality was 1.8% （2/112） and 0.5% （ 1/202 ） respectively in the emergency operation group and the selective operation group. Conclusions Individual treatment program combined with surgical operations should be used in the intra- and extra-hepatic calculus of bile duct in aged patients. And the selective operation is recommended strongly if possible.
