摘 要:目的探讨正常人左心室心肌扭转特点并分析其相关因素。方法对65例健康志愿者先行常规及多普勒超声检查,应用Simpson法获取左心室收缩末容积(LVESV)、左心室舒张末容积(LVEDV)、射血分数(EF);左心室长轴切面M型超声测量左心搏出量(SV)、左心室小轴缩短率(FS);频谱多普勒超声检测二尖瓣血流频谱,获得舒张早期E波加速度(E-acc)、E波减速时间(EDT)、E/A及E峰加速时间(E-AT)等。随后应用二维斑点跟踪成像(STI)技术定量分析胸骨旁心尖水平和心底水平的左心室扭转,观察左心室心肌扭转特点,并进一步探讨左心室扭转角度峰值与常规超声心动图测值参数的关系。结果正常人心尖水平心肌运动表现为逆时针旋转,基底部表现为顺时针旋转,左心室整体表现为逆时针旋转。常规超声测值中SV与左心室收缩末扭转峰值呈正相关(r=0.618,P=0.002),多普勒测值中E-acc和扭转角度呈显著正相关(r=0.953,P=0.004),EDT和扭转角度呈负相关(r=-0.950,P=0.003)。结论STI技术可以无创评估左心室心肌扭转运动,SV、E-acc和EDT与左心室扭转相关。Objective To evaluate the characteristics of left ventricular systolic twist (LVtw) and study its relative factors in healthy adults by two-dimensional ultrasound speckle-tracking imaging (STI). Methods Sixty-five healthy volunteers were enrolled and examined by two-dimensional ultrasound. As the echocardiographic parameters, End-systolic volume (ESV), End-diastolic volume (EDV) and Ejection fraction (EF) were collected by Simpson method. In addition, Stroke volume (SV), Short-axis fractional shortening (FS), Peak E velocity/Peak A velocity ( E/A), E-wave acceleration (E-acc), E-wave Descending time (EDT) and E-wave Acceleration time (E-AT) were measured. Then the standard LV basal, apical short-axis views and the values of LVtw were obtained using STI. The LVtw was defined as apical rotation relative to the base and observed in this study. The relationships between the LVtw and the echocardiographic parameters were further analyzed. Results As seen from the apex, the normal LV performed a clockwise rotation at the base and counterclockwise rotation at the apex . In our study, the SV was correlated positively to the LVtw (r = 0. 618 , P = 0. 002). The E-acc and LVtw had a positive relationship ( r = 0. 953, P = 0. 004). The EDT negatively related to the LVtw (r = - 0. 950, P = 0. 003). Conclusions The LVtw could be assessed by 2-dimensional ultrasound STI without any invasions. The SV, EDT and E-acc are correlated with the LVtw.
