经皮微创胸腔置管闭式引流术的护理体会

摘　　要：目的:探讨经皮微创胸腔置管闭式引流的护理方法。方法:对48例胸积液患者应用中心静脉导管行经皮微创胸腔置管闭式引流术,临床观察手术的安全性、术后并发症以及导管堵塞的预防措施。结果:48例患者46例一次穿刺置管成功,2例穿刺失败第二次置管,本组病人无发生术后出血、感染、气胸、胸液外渗等,导管滑脱1例,术后导管堵塞2例,堵管发生率4.16%。结论:微创胸腔置管闭式引流安全有效,但要加强置管护理,减少引流不充分、导管堵塞或术后并发症的发生。Objective： To approach the nursing measures of closed thoracic drainage percutem. Method：48 patients with pleural effusion were treated with closed thoracic drainage percutem by central venous cathe-ters. Then operation safety, postoperative complications and precaution of catheter jam were observed. Resuit： In all 48 patients, 46 patients were operated successfully one time, but 2 patients were treated trium-phantly by the second times. No postoperative hemorrhage, infection, pneumothorax and effusion from thorax occurred. But one patients happened catheter slippage and catheter jam occurred in two patients, the inci-dence rate of catheter jam was 4.16%. Conclusion： Closed thoracic drainage percutem safe and effective,but catheter nursing should be enhanced, deficient drainage , catheter jam and postoperative complications shouldbe reduced.
