摘 要:目的评估脐血IgE水平对婴儿过敏性疾病的阳性预测价值。方法采用荧光酶免疫法测定134例脐血中IgE的含量。出生后1个月、3个月、6个月、1年和3年电话或门诊随访,共5次(目前已完成前4次)。详细记录婴儿发生湿疹、反复哮喘和荨麻疹等各种过敏性疾病的发生及发展等情况。结果随访134例,随访率100%,有42例正常,有92例发生过敏反应,其中脐血IgE水平大于0.35 KU/L的婴儿发生过敏性疾病的为84例(68.7%),其中发生婴儿湿疹63例(47.0%),荨麻疹25例(18.7%),反复喘息12例(9.0%);脐血IgE水平小于0.35 KU/L的婴儿发生过敏性疾病的为8例(6.0%),发生婴儿湿疹3例(2.2%),荨麻疹4例(3.0%),反复喘息4例(3.0%)。结论结果提示脐血IgE水平升高的婴儿易患过敏性疾病,尤其是婴儿湿疹和荨麻疹。脐血IgE水平升高结合过敏家族史对婴儿过敏性疾病的预测价值较高。Objective Assessment of the the positive predictive value the infant umbilical cord blood IgE level has on allergic disease. Methods By using fluorescence enzyme immunoassay to measure cord blood IgE value of 134 cases. Born after 1 month, 3 months, 6 months, 1 year and 3 years, 134 ( 100% ) of the infants were followed up by telephone interview or were asked to come to clinic five times (completed 4 times till now) . Recording in details the actual condition of infants with eczema and repeated asthma and urticaria, or other allergic diseases. All data were obtained by using EXCEL software for statistical analysis. Results Among the 100% Follow - up 134 case interview, there are 42 cases appearing normal, 92 cases having allergic reactions, including 84 patients (68.7%) of cord blood IgE levels greater than 0. 35 KU / L of infants, in which occurred 63 eases (47.0%) of baby Eczema, 25 ea- ses ( 18.7% ) of urticaria, 12 cases (9.0%) of recurrent wheezing. Cord blood IgE levels of infants less than 0. 35 KU / L developed allergic disease for eight cases (6. 0% ) , in which there were three cases of infant eczema (2. 2% ) , four cases (3.0%) of urticaria, and four eases (3.0%) of recurrent wheezing. Conclusions The results suggested that cord blood IgE levels increased susceptibility of infants allergic diseases, particularly baby eczema and urticaria. Cord blood IgE levels connecting with family history has higher positive predictive values of allergic diseases of infant allergy.
