高频彩超诊断精索静脉曲张的临床价值

摘　　要：目的探讨高频彩超诊断各型精索静脉曲张患者各项检测指标的临床价值。方法应用高频彩超对25例正常对照组及99例男性不育患者左侧精索静脉在平静呼吸及乏氏动作时的内径、最大反流速度及持续时间进行检测。结果①正常对照组与病例组左侧睾丸体积差异有统计学意义(P<0.01);②正常对照组与病例组精索静脉内径、最大反流速度及持续时间差异均有统计学意义(P<0.01);③各病例组内精索静脉内径测值在平静呼吸与乏氏动作比较差异有统计学意义(P<0.01);④各病例组中随着内径的增宽,乏氏动作时其最大反流及持续时间比较差异有统计学意义(P<0.01)。结论高频彩超可为临床治疗男性不育患者提供精确的精索静脉内径、血流动力学及睾丸大小等客观指标,可作为精索静脉曲张患者常规的重要的无创检测手段。Objective To discuss the clinical value of indexes of color doppler ultrasound in diagnosis of patients with varicocele. Methods High frequency ultrasound was applied in the evaluation of diameter, maximum speed and duration of reflux of 25 patients in normal control group and 99 male infertility patients in experiment group. Results ①Size of left testis in the experiment group and normal group was significantly different （P〈0.01） ②Varicosity vein diameter, maximum speed and duration of reflux in experiment group and normal group were significantly different（P〈0.01） ;③Varicosity vein diameter, maximum speed and duration of reflux in a calm breathing were also significantly different between the different groups of patients （P〈0.01）; ④With the increase of the varicosity vein diameter, maximum speed and duration of reflux of the Valsalva experiment were significantly different （P〈0.01）. Conclusion High-frequency color Doppler ultrasound in the treatment of male infertility could provide objective indicators such as accurate varicosity vein diameter, hemodynamics and testicular size, which can be used as conventional and important means of non-inva-sive examination of patients with varicocele.
