摘  要:目的:探讨N-甲基-L-精氨酸(L-NMMA)对大鼠单侧睾丸扭转后对侧睾丸生精功能的保护作用。方法:42只健康雄性SD大鼠随机均分为假手术组、安慰剂组和L-NMMA组。建立左侧睾丸扭转模型。安慰剂组和L-NMMA组大鼠分别于扭转复位前30min通过尾静脉缓慢推注生理盐水和L-NMMA。三组大鼠分别于术后1周、2个月时处死7只,即刻采集各组大鼠未扭转侧睾丸标本,测定睾丸组织丙二醛(MDA)水平,同时进行组织病理学观察。结果:与安慰剂组相比,L-NMMA组1周时MDA值明显下降,平均曲细精管直径明显升高,而睾丸结构评分则明显偏低(P<0.05);2个月时主要表现在生精上皮层数和平均曲细精管直径较高,睾丸结构评分较低,两组间比较均有统计学差异(P<0.05)。结论:L-NMMA可以减轻大鼠单侧睾丸扭转后对侧睾丸的早期损害,对晚期对侧睾丸的生精功能具有一定的保护作用。Objective:To explore the biochemical and histological changes in the contralateral testis of rats with unilateral testicular torsion and elucidate the protective effects of L-NMMA on the contralateral testicular spermatogenetic function. Methods: Fouty-two prepubertal Sprague-Dawley rats were randomly divided into 3 groups, with 14 rats in each. The rats in sham-operated group underwent a sham operation of the left testis under general anaesthesia. The rats in saline group and L-NMMA group underwent a similar operation but the left testis was rotated 720°clockwise for 4h, maintained by fixing the testis to the scrotum, and saline/L-NMMA infused during half an hour before detorsion was performed. All the right (untwisted) testis were removed from rats at 1 week and 2 months intervals after surgery, 7 rats were taken from each group each time. Then the malondialdehyde (MDA) levels were evaluated, histology and seminif-erous tubule diameters were assessed. Results:Compared with saline group, the MDA levels in L-NMMA group decreased significantly while the mean seminiferous tubular diameter of contralateral testis and histological grades maintained in a better condition at 1 week interval, and it was also known that the rats in L-NMMA group got a higher score at the grades of germ cell layers and mean seminiferous tubular diameter of contralateral testis at 2 months interval. Conclusion: L-NMMA can reduce the damage of contralateral testis of prepubertal rats with unilateral testicular torsion during the early period, and L-NMMA may also have a protective effect on the testicular spermatogenetic function in the later stage.
