摘  要:目的:探讨原发性肝癌患者的CT与MRI影像表现特征,探讨用影像表现与PCNA蛋白及血清AFP阳性表达结果相对照,以判断肿瘤的生物学行为。方法:经手术切除并经病理证实的原发性肝癌29例作为研究对象,确诊的肝硬化27例作为对照。观察肝癌的CT或MRI表现特征,用电化学发光法检测血清AFP,免疫组织化学方法检测肿瘤细胞PCNA蛋白阳性表达率,并结合病理分级结果进行对照分析。结果:原发性肝癌PCNA的表达与病理分级相关,PCNA、AFP的表达与肿瘤大小和无包膜或包膜不完整均相关。原发性肝癌的增强类型与PCNA及AFP的阳性表达无明确关系。结论:原发性肝癌的CT和MRI影像中肿瘤体积增大及无包膜或包膜不完整在一定程度上反映肝癌的AFP、PCNA的表达,这些影像特征的出现提示肝癌恶性度高、易转移、预后不良。Objective: To investigate the relationship between the imaging findings on spiral computed topography and (or) magnetic resonance imaging and the expressions of proliferating cell nuclear antigen (PCNA) and serum alpha-fetoproteins(AFP) in primary hepatocareinoma. To study the possibility of reflecting the change of hepatocarcinoma molecular biology with the imaging findings,PCNA and AFP. Methods:Twenty-nine cases with primary hepatocarcinoma(PHC)were verified by histopathology SCT and/or MRI scanning.We employed immunohistochemical technique to estimate the positive expression rate of PCNA in tumor specimens,and CLEIA to measure human AFP in serum. The relationship between the imaging features and the positive expression rate of PCNA and AFP was observed.Results: It showed that there was close correlation between the positive expression rate of PCNA and pathological grade. The positive expression rates of PCNA and AFP were closely correlated with the diameter of lesion on image and incomplete pseudo-capsule. No correlation was found between the enhancement pattern and the expression of PCNA and AFP in hepatocarcinoma, Conclusion: Some SCT and MRI features of primary hepatocarcinoma such as the large tumor and incomplete pseudo-capsule may reflect the expression levels of PCNA and AFP. From these imaging features of the primary hepatocarcinoma, the bad be- havior and poor prognosis can be predicted.
