摘 要:目的:探讨MR各序列成像在脑出血性病变的敏感性。方法:回顾性分析100例脑出血患者常规自旋回波(SE)T1WI、快速自旋回波(TSE)T2WI、液体衰减反转恢复序列(FLAIR)、扩散加权成像(DWI)和小角度激发快速梯度回波序列(T2-FFE)资料,比较SE-EPI、T2-FFE序列脑出血病灶的显示率,分析其信号特征,并探讨SE-EPI代替T2-FFE的可行性。结果:T2-FFE为显示脑出血性病灶最敏感的序列,能显示常规序列所不能显示的微出血。无论在检出脑出血灶,还是图像对比度,SE-EPI均远低于T2-FFE。结论:T2-FFE序列能提高脑出血病灶的检出数及早期诊断率,应作为常规扫描序列;在临床脑出血筛选方面,SE-EPI无法代替T2-FFE。Objective:To evaluate the most sensitive MR sequences in the detection and diagnosis of the intracranial hemorrhagic lessons.Methods:Retrospectively, 100 cases with intracranial hemorrhagic lessons were performed with MRI routine scan,FLAIR,DWI and T2-FFE sequences. The sensitivity of SE-EPI and T2-FFE in the hemorrhagic lessons was compared to determine whether the EPI sequences can be used as an alternative to the GRE sequence for detection of chronic hemorrhage. Results:T2-FFE was the most sensitive sequence in the detection of intracranial hemorrhagic lessons. T2-FFE could detect microbleeds which couldn't be displayed in the routine sequence, such as T1WI, T2WI, FLAIR. Compared to SE-EPI, T2-FFE had a better sensitivity, and the figure contrast was better also. Conclusion:T2-FFE sequence played an important role in the detection and diagnosis of intracranial hemorrhagic diseases, which made it necessary for the clinical MR examinations. And SE-EPI could not use to replace T2-FFE in the detection of hemorrhagic lessons, especially microbleeds.
