摘  要:目的比较0.375%左旋布比卡因与0.375%罗哌卡因用于臂丛神经阻滞的临床效果。方法50例ASAI~II级行择期上肢手术患者,随机分为左旋布比卡因组(L组)、罗哌卡因组(R组),各25例。分别用0.375%左旋布比卡因和0.375%罗哌卡因30mL行肌间沟臂丛神经阻滞。监测并记录麻醉前(T0)、麻醉后即刻(T1)、麻醉后5min(T2)、10min(T3)、20min(T4)、30min(T5)的HR、MAP、SpO2、RR,观察两组的麻醉起效时间、术后感觉及运动功能恢复时间和麻醉镇痛质量。结果两组在各时间段的HR、MAP、SpO2、RR差异无统计学意义。麻醉起效时间:L组为(4.6±1.2)min,R组为(4.9±1.4)min(P>0.05);感觉和运动功能恢复时间:L组为(485±l40)minvs(220±30)min,R组为(445±130)minvs(186±38)min(P>0.05)。两组麻醉镇痛质量相似,差异无统计学意义。结论0.375%左旋布比卡因与0.375%罗哌卡因30mL皆可用于臂丛神经阻滞,两者麻醉起效时间、神经功能恢复时间及麻醉镇痛质量相似。Objective To compare the clinical efficacy of 0.375% levopubivacai'ne and ropivacaine used in brachial plexus block. Methods Fifty ASA Ⅰ-Ⅱ patients undergoing upper limbes operation under brachial plexus block were randomly divided into two groups,group L(n=25) and group R(n=25). Patients received interscalene brachial plexus block with 0.375% L or 0.375% R 30mL HR,MAP,SpO2,RR during operation were recorded. The onset time,the time to recovery of sensory and motor funetion and the efficacy of anesthesia were observed. Results There was no significant difference between group L and group R in HR,MAP,SpO2,RR during operation. The onset time of surgical anesthesia was (4.6 ± 1.2)min with levopivacaine and (4.9 ± 1.4)min with ropivacaine (P〉0.05). The time to recovery of sensory and motor function was (485 ± 140)min vs (220±30)min in group L and (445 ± 130)min vs (186 ± 38)min in group R, respectively(P〉0.05). Both group L and group R could provide the similar efficacy of analgesia. Conclusion Both 0.375% levobupivacaine and ropivocaine used in brachial plexus block can provide comparable surgical anesthesia.
