双肾GFR、ERPF及血浆肾功能指标对糖尿病患者肾脏功能的评价

摘　　要：目的：用血浆肾功能指标和核医学检查评价糖尿病肾功能。方法：同周查ＧＦＲ、ＥＲＰＦ和血糖、尿素氮、肌酐及尿蛋白，正常人１０例，ＩＤＤＭ和ＮＩＤＤＭ病人３３例。数据经ｔ检验处理。结果：ＩＤＤＭ组血糖和ＧＦＲ最高（ｔ＝６．００５，Ｐ＜０．０ｌ）；ＮＩＤＤＭ肾病组ＧＦＲ、ＥＲＰＦ明显降低（ｔ＝４．１８，Ｐ＜０．０ｌ），其它高于正常。ＮＩＤ－ＤＭ组和ＮＩＤＤＭ微蛋白尿组仅有微量尿蛋白和高血糖，余接近正常。结论：ＳＰＥＣＴＧＦＲ在糖尿病早期增高，但时间较短，当控制血糖和治疗后可接近正常，尿蛋白在ＧＦＲ稳定期，可作为肾功监测指标，其它则不能作为早期监测指标。Objective:Plasma indicators and nuclear medicine tests are used to detect the renal function of Diabetes melli-tus.Methods:To check the same patiend Glomerular filtration rate(GFR),mffective Renal Plasma Flow(ERPF)Glucose uieanitrogen(UN)creatinine and protein urine in a week,normal patient 10 Non-insulin-dependent diabetes mellitus(NIDDM)and Insudin－dependent diabetes mellitus(IDDM)patient 33．Dates are processed by t test Results:In IDDMgroup,Glucose and GFR are the highest in every group(t=6.005，P＜0．01),In NIDDM group GFR and ERPF are bothgreatly decrease(t=4．18,P＜0．01),UN creatinine and protein urine are higher than any other group,In NIDDM group and NIDDM minoor-protein urine GFR ERPF、UN and creatinne are normal,only having minor-protein urine.Conclusion:SPECT GFR is greatly high at early stage of diabetes mellitus,this procedure is transient, coming to normal with controlling glucose and effective trestment,protein urine may be a detector of renal function in GFR constant stage;While weanitrogen,creatinine and ERPF couldnt being a indicatorof early stage in renal function damage,
