病例组合模式的系统化研究

摘　　要：目的:完善医院信息化建设和财务管理,推动病例组合模式应用于我国的卫生管理领域。方法:收集国内外相关资料,研究发达国家病例组合模式的应用情况和经验,分析我国实施病例组合模式的应用环境。结果:支持病例组合管理模式的运行需要各级医疗机构建立符合病例组合要求的信息化系统和医院成本核算体系,目前我国医院信息化建设中缺乏信息标准化的管理,系统的互操作性差,另外我国各级医院尚未建立完善的成本核算中心。这些都阻碍了我国实施病例组合管理模式。结论:病例组合无疑为我国的卫生改革提供了新的思路和客观卫生管理标准,为了实施病例组合的卫生管理模式,我国有必要借鉴国外卫生管理成功的经验,结合中国的国情,建立起一个统一可行的"病例组合"实施方案,按照病例组合要求健全我国的医院信息化系统和财务核算管理体系。Objectives：to perfect hospital information infrastructure and the financial management, to promote the implementation of case-mix in health care management in China. Methods.. to collect the domestic and foreign case-mix related information, to study practice and experience of case-mix in advanced country and analyze the application environment or infrastructure of case-mix in our country;Results：to introduce case-mix system into our health care management, we need establish hospital information system and cost accounting system which conform to the requirements of case-mix. However Our Hospital information systems which use little health information standardized design and terminology are not interoperable. In addition, we have not established perfect case-mix cost centers. These make it difficult to implement case-mix in our health management. Conclusions：The case-mix provides a new approach and an objective standard of health management in our health system reform. To implement case-mix, the successful experience of other countries in health care management is considered to be referred and a feasible plan （including development of hospital information systems and hospital financial management） for the implementation of case-mix in our health care management be build.
