老年高血压病动态血压水平与血浆内皮素水平关系的临床研究

摘　　要：为了解内皮素在老年人高血压病中的作用，测定了６９例老年高血压病患者及２０例正常对照者血浆内皮素浓度。采用偶测及动态血压监测方法同时测量血压，内皮素水平采用放免法测定。结果显示：①Ⅱ期（７６．１１±１５．３９）×１０－９ｇ／Ｌ、Ⅲ期（７９．９３±１２．３７）×１０－９ｇ／Ｌ高血压患者血浆免疫活性内皮素水平明显高于Ⅰ期高血压患者（６６．０９±１２．５２）×１０－９ｇ／Ｌ；Ｐ＜０．０５，及正常组（５９．２９±１２．９９）×１０－９ｇ／Ｌ。②血浆内皮素与血压值存在弱正相关，Ｐ＜０．０５。结果表明：老年人高血压病血浆内皮素水平升高与血管并发症相关；在与内皮素相关方面，动态血压值优于偶测血压值。To investigate the role of endothelin in elderly essential hyertension, we measured plasma endothelin in 69 elderly essential hypertension patients and 20 control subjects. Blood pressure was assessed by both casual and noninvasive 24hour ambulatory blood pressure monitoring, and endothelin levels were measured by radioimmunoassay. The results showed that: ①Plasma irET levels in patients of stage Ⅱ[(76.11±15.39)×10-9 g/L, n=23] and stage Ⅲ[(79.93±12.37)×10-9 g/L, n=15] were significantly higher (P<0.05) than that in patients of stage Ⅰ[(66.09±12.52)×10-9 g/L, n=31] and normal control subjects [(59.29±12.99)×10-9 g/L, n=20]; ②There was a weak but positive correlation between plasma irET levels and blood pressure values (24hDBP, r=0.257;dDBP,r=0.274;dSBP,r=0.256;nSBP,r=0.212;cDBP,r=0.236;P<0.05). These findings suggested: ①The elevated plasma levels of endothelin in elderly patients with essential hypertension is related to vascular complications. ②ABPM mean values are better than causal BP values in the correlation with ET.
