含奥沙利铂的化学治疗方案治疗晚期头颈部癌的疗效

摘　　要：目的评价含奥沙利铂的化学治疗(化疗)方案治疗晚期头颈部癌病人的疗效和安全性。方法50例晚期或复发转移性头颈部癌病人分别接受含奥沙利铂的化疗方案和含顺铂的化疗方案。治疗组(25例):奥沙利铂130 mg·m^(-2),静脉滴注d 2;氟尿嘧啶750 mg·m^(-2),静脉滴注,d 1～5;21 d为一个周期;对照组(25例):顺铂80 mg·m^(-2),分2～3 d给予(配合水化);氟尿嘧啶750 mg·m^(-2),静脉滴注,d 1～5;21 d为一个周期。结果治疗组:1例(4%)达完全缓解,12例(48%)达部分缓解,有效率为52%(13/25)。中位疾病进展时间5.4 mo,1年生存率为40%。对照组:无完全缓解病例,11例(44%)达部分缓解,有效率为44%(11/25)。中位疾病进展时间4.2 mo,1年生存率为32%。2组差异无显著意义(P>0.05)。主要不良反应如肾功能损害、血液毒性2组均无显著差异。结论含奥沙利铂的化疗方案是治疗晚期或复发转移性头颈部癌安全、有效的联合化疗方案。AIM with advanced head and and neck cancers were To evaluate the efficacy and toxicity of chemotherapy involving oxaliplatin in patients neck cancers. METHODS Fifty patients with advanced, recurrent or metastatic head equally divided into treatment group and control group. The former was treated with oxaliplatin 130 mg·m^-2, iv, gtt, on d 2 and fluorouracil 750 mg·m^-2, iv, gtt, on d 1-5 taking 21 days as one cycle; and the latter was trated with cisplatin 80 mg·m^-2, iv, gtt, on d 1-3 and fluorouracil 750 mg·m^-2, on d 1-5, using 21 days as one cycle. RESULTS One （4 % ） of 25 patients in treatment group showed complete response and 12 （48 %） partial response with an overall response rate of 52 % （13/25）. Median time of progression was 5.4 months, and 1-year survival was 40 %. No patients of 25 patients in control group showed complete response and 11 （44 % ） partial response with an overall response rate of 44 % （11/25）. Median time of progression was 4.2 months, and 1-year survival was 32 % ; with no statistically difference between the two groups （P 〉 0.05）. Major toxicities including gastrointestinal tract reaction, renal damage and peripheral neurotoxicity showed no significant difference （P 〉 0.05）. CONCLUSION The chemotherapy involving oxaliplatin is a safe and effective combination treatment for patients with recurrent and/or metastastic head and neck cancer.
