严重烧伤患者早期应用重组人生长激素的临床效果及安全性分析

摘　　要：目的观察早期应用重组人生长激素(rhGH)治疗严重烧伤患者的有效性及安全性。方法将伤后3d内入院的严重烧伤患者随机分为治疗组和对照组,每组23例。治疗组从伤后第3天开始每日给予rhGH0.3IU/kg皮下注射,持续14d。对照组则每日同法注射等量等渗盐水。治疗期间定期检测患者的一般情况、蛋白质代谢、糖代谢、肿瘤坏死因子(TNF-α)、白细胞介素6(IL-6)、电解质的变化。结果与对照组相比,rhGH治疗患者一般情况良好,体重下降减少;血浆蛋白水平明显升高(P<0.05);血糖水平升高(P<0.05),但未出现难以控制的高血糖患者;TNF-α,IL-6水平显著降低(P<0.01);24h尿中Na+,Cl-含量显著减少(P<0.05)。结论rhGH可调节严重烧伤患者的高代谢反应,有效促进蛋白质合成,改善营养状况,增强免疫功能,降低全身感染机会,提高存活率,缩短住院时间,临床应用较为安全,疗效可靠。Objective To observe the efficacy and safety of recombiuant human growth hormone（ rbGH） iu severely burned patients. Methods Furry-six cases of severely burned patients admitted within 3 days were randomly divided into rhGH group and comtrol group. For rhGH group, rhGH was given subcutaneously in the dosage of 0.31U·kg^-1 each nigbt for 14 days. Normal saline was given to the control group. Protein metabolism,glucose metabolism,TNF-α,IL-6 and electrolyte changes were assayed. Results Conparing with control group,the patients of the rhGH group had a better general condition, less decrease in body weight, higher protein and blood sugar level in plasma（ P 〈0.05） , lower level of TNF-α and IL-6 and less output of Na ^＋ and CI^- in urine during 24 hours（ P 〈0.05）. Conclusions rhGH cau effectively adjust hypennetabolism, promote protein synthesis, enhance immunity, decrease the incidence of iufection, and reduce the length of hospitalization of severe burned patients, h has secure and steady effect in clinical application.
