摘 要:目的观察汉防己甲素预防椎板切除术后硬膜外粘连的效果。方法制作动物模型,以汉防己甲素为植入材料,并设阳性及空白自身对照,在不同时相点行大体、显微观察及MRI检测。结果椎板切除后硬膜粘连发生的高峰在8周以内,汉防己甲素能抑制椎板切除术后硬膜外粘连,4周以内效果与游离脂肪移植效果相近,8周以后的效果则强于游离脂肪移植,在观察时间内没发现明显不良反应。结论汉防己甲素能有效地降低椎板切除术后硬膜外粘连,且无明显不良反应。Objective To investigate the effect of prevention of tetrandrine on peridural fibrosis and adhesion after laminectomy. Methods Tetrandrine was used in animal model of laminectomy with positive and blank control. The experimental result was measured with double blind protocol by gross anatomical appearance, microscopical observation and enhanced MRI at 2,4,8 and 12 weeks postoperatively. Results The peak of peridural fibrosis and adhesion after laminectomy was found within 8 weeks postopera- tively. Tetrandrine could inhibit peridural fibrosis and adhesion after laminectomy effectively. The effect was similar to free fat transplantation within 4 weeks postoperatively but better after 8 weeks,without toxical side effect. Conclusion Tetrandrine can in- hibit peridural fibrosis and adhesion after laminectomy effectively.
