摘 要:【目的】探讨在博莱霉素(BLM)诱导的肺间质纤维化过程中,白细胞介素4(IL-4)的动态表达、分布及意义。【方法】将小鼠随机分成2组,实验组气管内注射BLM,对照组注射生理盐水。收集右肺支气管肺泡灌洗液(BALF)及灌洗细胞,左肺病理切片,另行肺组织原代培养,测定这些标本IL-4的表达。【结果】①在BLM诱导的肺间质纤维化过程中,IL-4在肺间质细胞、肺泡上皮细胞、肺泡及肺间质免疫细胞中广泛表达,且在病程的早期较强,随着病程的进展,其表达强度逐渐减弱。终末细支气管上皮细胞表达IL-4亦明显增强,但在时间上与其他细胞明显不同步。②实验组13、d支气管肺泡灌洗细胞IL-4表达最强,明显高于0.5、7、142、8 d。③实验组0.5、1、3d,BALF中IL-4的含量分别较对照组明显增高(P值分别为:0.002、0.003、0.006)。④实验组肺组织培养细胞IL-4表达较对照组明显增高(P<0.001)。【结论】在肺间质纤维化病理过程的早期,IL-4在肺组织中呈现广泛的过度表达,其中肺间质细胞IL-4表达增强,可能对促进纤维化的形成具有更重要的意义。[Objective] To investigate the dynamic expression, distribution and role of interleukin-4 (IL-4) in the lung tissues of the mice with bleomycin(BLM)-induced pulmonary fibrosis. [Methods] C57BL/6 mice were randomly divided into 2 groups, the experimental group and the control group, and they were treated with BLM-A5 (5mg/kg) or normal saline by intratracheal injection respectively. The mice in the control group were sacrificed on day 0.5, 1, 14 and the experimental group were sacrificed on day 0.5, 1, 3, 7, 14, 28 after intratracheal instillation. Bronchoalveolar lavage was performed 3 times in right lung. IL-4 immunohistochemistry stains were performed on the lavage cells and image analysis conducted. IL-4 immunohistochemistry stains were also performed on the pathological sections of the left lung tissues. On the third day after intratracheal administration, another 5 mice of each group were sacrificed and primary culture of the lung tissues were performed. IL-4 immunohistochemistry stains were performed on cultured cells and image analysis conducted. The average integral optical density was used to express the amount of IL-4. [Results]①IL-4 was generally expressed in the interstitial cell, alveolar epithelial cell, alveolar and interstitial immunocyte of lung, and the expression was more stronger in early phase, and taper along with the course of BLM-induced pulmonary fibrosis, and the expression of IL-4 in the tracheal epithelium cells was also increased, but it was not synchronized in the time course with other cells. ②The expression of IL 4 in BAL cells in the experimental group was significantly higher than that in the control group, and at 1,3 d after intratracheal instillation of BLM, the expression of IL-4 was significantly higher than the days of 0.5, 7, 14, 28 in the same group. ③The expression of IL-4 in BALF in'the experimental group was significantly higher than that in the control group at the days of 0.5, 1, 3, respectively (P were 0. 002, 0. 003, 0. 006, respectively).④T
