摘 要:目的探讨中老年妇女腰椎和髋部骨矿物质密度(BMD)的影响因素,以及不同测量部位的骨质疏松症(OP)检出率,为本市诊断和预防OP提供科学的参考。方法对南昌大学第二附属医院骨密度测定室检测的门诊和住院患者中随机抽取297例中老年妇女,采用双能X线骨密度吸收测定仪(DEXA)检测腰椎(L2-L4)、股骨颈、Wards′三角以及大转子的BMD,并进行统计分析。结果各部位BMD均随年龄的增加而下降,尤以Wards′三角下降最为明显。低体重指数组与超体重指数组比较,髋部各部位BMD差别有统计学意义(P<0.01)。腰椎(L2-L4)BMD与身高、体重指数呈正相关(P<0.05),髋部各部位BMD值均与身高、体重及体重指数呈正相关(P<0.01)。腰椎为中老年妇女OP检出率最高部位,其次为Wards′三角。结论各部位骨密度均随年龄增加而下降,髋部BMD与身高、体重及体重指数呈正相关,腰椎和Wards′三角可能为诊断OP敏感部位。Objective:To explore the variation regularity and influence factors of bone mass density (BMD) of lumbar spine and hip in middle aged and elderly female, and the effects of different measurement sites on the diagnosis of osteoporosis, then provide the scientific reference for diagnosing and prevention osteoporosis . Methods:297 middle aged and elderly female were sampled randomly from the cases who were examined in the bone mass density measurement department of The Second Affiliated Hospital of Nanchang University, The BMD of lumbar spine(L2-L4),femoral neck, Ward's triangle and great trochanter was measured by Dual Energy X-ray Absorptiometer(DEXA). Then all data were analyzed by SPSS. Results: BMD values of all sites decreased with the increasing of age, and the Ward's triangle declined most obviously. There were significant difference(P〈 0.01)in BMD values of hip between low BMI groupand high BMI group.The BMD values of lumbar spine (L2-L4) had positive correlation with stature and body mass index (BMI)(P〈0.05)and the BMD values of all sites of hip had positive correlation(P〈 0.01)with stature,body weight and BMI. The detectable rate of osteoporosis was the highest in lumbar spine, and then in Ward' s triangle. Conclusions:BMD values of all sites decrease with increasing of age, hip BMD values had positive correlation with stature, body weight and body mass index, and the sensitive sites of diagnosis osteoporosis were lumbar spine and Ward's triangle.
