生活满意度与自杀未遂关系

摘　　要：目的了解自杀未遂人群不同时期生活满意度并与对照组人群生活满意度比较,探讨生活满意度在自杀未遂中的作用。方法采用配对病例对照研究方法,选取综合医院急诊科或内科病房收治的自杀未遂者220例,对照人群选取无自杀行为史,年龄±3岁、同性别、相同居住地的其他病例或健康者。生活满意度量表采用WHO的自杀行为多中心干预研究量表中的生活满意度部分。比较2组人群不同时期的生活满意度;生活满意度从非常满意~非常不满意分为5个等级评分。结果自杀未遂者组以现在的生活满意度得分最高,平均为（3.24±0.91）分,5年前得分最低,平均为（2.70±0.82）分。单因素条件Logistic回归模型显示,不同时期生活满意度皆与自杀未遂的相关性差异有统计学意义,其中,现在生活不满意的OR=5.75,95%CI=3.11～10.61;1月前生活不满意的OR=6.0,95%CI=3.17～11.36;1年前生活不满意的OR=6.14,95%CI=2.76～13.66;5年前生活不满意的OR=2.27,95%CI=1.12～4.62;1年后生活不满意的OR=5.11,95%CI=2.50～10.44;5年后生活不满意的OR=7.33,95%CI=3.12～17.21。结论对生活不满意,尤其是对1月前及自杀未遂当时生活不满意对自杀未遂的发生有着一定的影响。Objective In order to understand the life satisfaction of different period of suicide attempt and compare them between suicide attempters and paired controls to explore the effect of life satisfaction on suicide attempt. Methods A paired case - control study was carried out with 220 suicide attempters who were gotten from departments of emergency or internal medicine of hospitals. The controls were selected from patients with other diseases or healthy persons who were in the same district, gender and age（within 3yrs） by 1：1 matching. And the controls were required not having history of suicidal behav- ior. The questionnaire including life satisfaction was referenced to multicenter intervention study on suicide behavior of World Health Organization. Life satisfaction was scaled from very satisfaction equal to 1 to very dissatisfaction equal to 5. The differ- ent periods of life satisfaction were now, 1 year after, 5 years after, 1 month before, 1 year before, 5 years before. Results During different periods of life satisfaction of suicide attempt, there were highest score in the period of suicide attempt time, its mean and standard deviation were 3.24 and 0.91 and the lowest score in the period of 5 years before suicide time, its mean and standard were 2.70 and 0.82. Single variable conditional logistic regression model showed that life satisfactions of different periods of suicide attempt were all significantly associated with suicide attempt and the Odds Ratios （95% CI） were followed as below： life dissatisfaction of suicide attempt time being 5.75 （95 % CI：3.11-10.61 ）, one month before being 6.0 （95% C1：3.17-- 11.36）, one year before being 6.14 （95% CI ： 2.76 ～ 13.66）, five years before being 2.27 （95% CI ： 1.12 ～4.62）,one year after being 5.11 （95%CI：2.50～10.44）,five years after being 7.33 （95% CI：3.12-- 17.21）.Conclusion Life dissatisfaction, especially life dissatisfaction of one month before and suicide attempt time might play a certain role in suieide
