影响女性阴道乳酸杆菌数量分布的相关因素分析

摘　　要：目的研究年龄、月经周期、行为因素及不同感染状态对阴道乳酸杆菌数量的影响。方法对2005年10月至2006年3月来北京大学第一医院妇科门诊进行体检的197例妇女阴道分泌物进行滴虫、假丝酵母菌、细菌性阴道病检测及阴道乳酸杆菌计数。并取宫颈分泌物进行衣原体检测及宫颈脱落细胞进行HPV检测。结果随着年龄的增长,阴道乳酸杆菌数量发生变化,>50岁年龄组阴道乳酸杆菌检出率明显减少、阴道炎发生率增加。细菌性阴道病患者阴道乳酸杆菌计数(4×106/L)明显减少,与正常人(5×1010/L)相比差异有统计学意义(P(0.05)。外阴阴道假丝酵母菌病患者阴道乳酸杆菌计数(1.3×107/L)与正常人相比差异亦有统计学意义(P(0.05)。结论阴道乳酸杆菌的数量分布受年龄及阴道感染的影响,月经周期及行为因素对乳酸杆菌的影响有待进一步研究。Objective To investigate the influence of age, menstruate period, behavior and vaginitis on the distribution of lactobacilli in vagina. Methods Totally 197 women＇s vagina secretions were collected for trichomonas, monilial, and bacterial vaginopathy testing as well as vagina lactobacilli counting. They were outpatients of Department of Obstetrics and Gynecology, Peking University First Hospital during Oct. 2005 to Mar. 2006. Cervix secretions were collected for Chlamydia test. HPV test was done to exfoliate cells from cervix. Results The number of lactobacilli decreased when the age grew. A significant decrease in lactobacilli and an increase in vaginitis were found in the group of people above 50 years old. There was no significant difference in the number of lactobacilli among the women with different menstruate periods （P 〉 0.05）. An important decrease of lactobacilli was found in the patients with bacterial vaginopathy, which was significant compared with normal women （P 〈 0.05 ）. The difference of lactobacilli count between patients with vulvovaginal candidosis and normal women was also significant （P 〈 0.05 ）. No significant difference was found between HPV and Chlamydia patients （P 〉 0.05）. Conclusion Age and vaginal infection influence lactobacilli distribution in vagina. The influence of menstruate period and behavior on lactobacilli distribution in vagina needs a further study.
