妊娠合并血小板减少的临床分析

摘　　要：目的探讨妊娠合并血小板减少的病因及围生期的处理方法。方法分析2000-2007年佛山市第一人民医院65例妊娠合并血小板减少患者的临床资料。结果65例患者中妊娠相关性血小板减少(PAT)30例,妊娠期高血压所致血小板减少18例,特发性血小板减少性紫癜(ITP)10例;其他原因7例。阴道分娩15例;剖宫产50例产后出血4例,3例为剖宫产术中宫缩乏力所至,经处理后效果良好。1例为胎盘植入,行全子宫切除术。4例HELLP综合征,3例胎死宫内,1例新生儿重度窒息死亡。1例新生儿早产重度窒息经抢救痊愈。结论该病是由多种内科合并症及妊娠并发症引起的一种围生期常见病,处理不当,将危及母胎生命,应针对病因进行个体化处理。分娩方式视血小板情况及有无产科手术指征而定。Objective To investigate the cause and perinatal treatment of pregnancy with thrombocytopenia. Methods We retrospectively analyzed 65 cases of pregnant women with gynecological thrombocytopenia with the clinical data 2000 -2007, the First People ＇s Hospital of Foshan City. Results Total 65 cases of patients with pregnancy - associated thrombocytopenia （PAT） 30 cases; gestational hypertension- induced thrombocytopenia in 18 cases; idiopathic thrombocytopenic purpura （ITP） in 10 cases; Other causes 7 cases. 15 cases of vaginal delivery; cesarean section 50 cases of postpartum hemorrhage four cases; and 3 cases of cesarean section in the uterine atomy in those, treated well. One case of placenta accreta, to hysterectomy. Four cases of HELLP syndrome, and three cases of intrauterine fetal death, and one case of neonatal severe suffocation and death, one case of premature newborns severe asphyxia recovered by the rescue. Conclusion Pregnancy complicated by the variety of platelet reduce medical complications arising from pregnancy complications and perinatal is a common disease, handled not properly, will threaten maternal -fetal life, the individual should be a cause for treatment. Delivery way is depended on platelets and surgical indications.
