摘 要:目的探讨参麦注射液对鼠布比卡因中枢神经系统及心脏毒性的影响。方法20只SD大鼠随机分为对照组和参麦注射液预先给药组(研究组)。在静脉泵注布比卡因2mg·kg^(-1)·min^(-1)前30 min,研究组腹腔内注射参麦注射液10 mL/kg,对照组予等量0.9%氯化钠溶液。持续监测平均动脉压和心率,记录大鼠抽搐、心律失常、心搏停止时的布比卡因用量。结果两组大鼠出现抽搐及心律失常时的平均动脉压均显著高于泵注布比卡因前(P值均<0.01),心律失常时的心率均显著慢于泵注布比卡因前(P值均<0.05),相同情况下两组间平均动脉压和心率的差异均无统计学意义(P值均>0.05)。研究组大鼠出现抽搐、心律失常及心搏停止时的布比卡因剂量分别为(8.1±2.8)、(7.7±2.7)和(20.6±3.5)mg/kg,显著大于对照组的(5.7±0.9)、(6.9±1.1)和(17.6±3.4)mg/kg(P值分别<0.05、0.01)。结论预先给予参麦注射液可减轻静脉注射布比卡因对鼠中枢神经系统及心脏的毒性。Objective To study the influence of Shenmai injection, a preparation of Chinese herbal medicine, on bupivacaine associated toxicity to the central nervous and cardiac system in rats. Methods Twenty SD rats were randomly assigned to 2 groups: control group and Shenmai injection pretreatment group (study group). Mean arterial pressure (MAP), heart rate (HR) and electrocardiography were continuously monitored. Animals in 2 groups received continuous infusion of bupivacaine at 2 mg·kg^-1·min^-1. Shenmai injection (10 mL/kg) was given intraperitoneally 30 min before bupivacaine infusion whereas in the control group equal volu me of normal saline was injected instead. The duration between the beginning of bupivacaine infusion and onset of convulsion/arrhythmia (QRS≥90 ms)/asystol was recorded and the amount of bupivacaine infused was calculated. Results The MAP values were significantly increased (P〈0.01) and the HR was significantly decreased (P〈O. 01 ) after bupivacaine infusion and the appearance of convulsion and arrhythmia in both groups. There was no significant difference in the MAP and HR at the same conditions between the 2 groups (P〉0. 05). In the study group, the amounts of bupivacaine causing convulsion, arrhythmia and asystol were (8. 1 ± 2. 8), (7.7±2.7) and (20.6 ± 3.5) mg/kg, respectively, which were significantly higher than those in the control group ([5.7±0.9], [6.9±1. 1] and [17.6±3.4] mg/kg, respectively, P〈0. 05 or 0.01). Conclusion Pretreatmaent with Shenmai injection can reduce the toxicity of bupivacaine to the central nervous and cardiac systems in rats. (Shanghai Med J, 2008, 31:346-348)
