摘 要:对6例行原位肝移植术患者,在无肝期采用左侧股静脉和门静脉至左侧腋静脉的体外静脉-静脉转流,转流量为400~800ml/min。围术期分别在转流前、转流期30分钟和60分钟、开放后新肝期30分钟、术毕等采动脉血作酸碱和生化检查。结果显示:BE在转流60分钟有所下降(P<O.05),Ca2+在开放后30分钟显著下降(P<0.05),其它指标如pH、K+等变化无显著差异(P>0.05)。表明在体外静脉-静脉转流下进行肝移植,避免了无肝期和开放期血液酸碱和生化的急剧变化,能减少或避免围术期代谢性酸中毒和高钾血症。Six patients received orthotoptic liver transplantation.Veno-venous bypass was performed in enheptic phase. The rate of rotation 400~800ml/min. The profiles of acid and biochemical changes in perioperation were obtained. The results showed:BE slight decreased in 60 minutes on bypass(P <0. O5 ); the serum Ca2 + decreased greatly after the hep-tic revascularization (P < O.O5. pH and serum K+ and or sohad no changes (P > 0.05 ). It was concluded that the clinical orthotoptic liver transplantation under veno-venous bypass could avoid greatly acid- base and biochemical changes the group of heptic transplantation avoided obvious metabolic aci-dosis and hyperkalemia.
