早期微创性磨削术治疗颜面部深Ⅱ度烧伤的临床研究

摘　　要：目的探讨早期微创性磨削治疗颜面部深Ⅱ度烧伤的效果。方法将52例颜面部深II度烧伤患者随机分为磨削组和对照组。磨削组患者于伤后24小时内以钢丝球磨削,以油纱、0.02%碘伏湿敷包扎,术后3天连续换药;对照组以0.02%碘伏包扎进行换药治疗。分别观察消除肿胀、创面愈合时间和愈合质量等情况,并随访观察瘢痕增生情况。结果在消除肿胀方面,磨削组在伤后第1、2及第5天与对照组无差别(P>0.05),但在伤后第3、4天优于对照组(P<0.05)。磨削组创面平均愈合时间(15.00±1.41)天,较对照换药组(23.96±2.22)天均缩短(P<0.05)。磨削组在愈合质量方面,疗效明显优于对照组(P<0.05)。结论早期磨削治疗颜面部深Ⅱ度烧伤,不受面部凹凸处的限制,符合整形外科微创手术的技术要求,提高了创面愈合质量,是一种手术治疗颜面部深II度烧伤的有效方法。Objective To explore the results of repair of deep -Ⅱ facial bum wounds with dermabrasion . Methods Fifty - two patients with deep - Ⅱ facial bums were enrolled in the study and were randomly divided into 2 groups. One group was served as the experimental group receiving dermabrasion treatment, and the control group was dealed with the routine treatment. The capacity of eliminate necrotic tissue and strut, the healing time and the prognosis were compared between the two groups. Results The method of experimental group effectively eliminated strut at the 3rd and 4th post bum day than that in the routine treatment （ P 〈 0. 05）. The average healing time was 15. 00 ± 1.41 days after dermabrasion, which was shorter than that in control group 23.96±2.22 days（P 〈0. 05）. Curative effect of dermabrasion group in wounded healing and cheloid was better than that in the control group （ P 〈 0. 05）. Conclusion The dermabrasion is simple and effective technique for treating deep - Ⅱ facial bum. It can promote the healing quality to the wounds.
