摘 要:[目的]研究呼吸机相关性肺炎(VAP)病原菌临床流行病学分布及耐药性,为防治VAP提供科学依据。[方法]前瞻性调查2007年1~12月重症监护病房VAP患者,分析VAP患者临床资料、病原菌谱及耐药性。[结果]236例机械通气患者有98例发生VAP,VAP发生率为41.52%,共分离出133株病原菌,其中革兰阴性菌占72.04%,铜绿假单胞菌居于首位;革兰阳性菌占20.43%;真菌占7.53%。[结论]VAP病原菌以革兰阴性杆菌为主,并有多重耐药;革兰阳性球菌对万古霉素敏感,临床医师应根据药敏试验结果合理使用抗菌药物,及时治疗VAP。[Objective]To study the distribution and antimicrobial resistance of bacteria causing ventilator-associated pneumonia (VAP) in order to provide evidence for prevention and therapy of VAP. [Methods]Prospective study was conducted on VAP patients from January to December, 2007. [Results]Totally 236 patients were enrolled in this study, of which 98 were diagnosed as VAP. The morbidity of VAP was 41.52%, A total of 133 pathogens were identified. Among all the strains casing VAP, Gram-negative bacteria accounted for 72.04%,in which Pseudomonas aeruginosa was the major pathogen. Gram positive cocci accounted for 20.43% and 7.53% were fungi. [Conclusion]Gram-negative bacteria were the most common pathogens of VAP,which presented to multi-drug resistant pattern. Gram positive cocci to vancomycin were all 100%. Clinician should choose antimicrobial agents according to antimicrobial susceptibility test result in time.
