女性盆底器官脱垂患者主韧带超微结构的研究

摘  要:目的通过透射电镜观察主韧带的超微结构,同时测量主韧带胶原原纤维直径,探讨盆底器官脱垂的发病机制。方法选取深圳市龙岗中心医院妇科2006年6月至2007年6月确诊为盆底器官脱垂行阴式手术的10例患者为实验组,同期因子宫良性病变行阴式子宫切除术的10例患者为对照组。术中取主韧带标本,通过透射电镜观察主韧带超微结构的变化,并测量主韧带胶原原纤维直径。结果①实验组主韧带胶原束排列紊乱,呈扭曲状,可见断裂结构,由结缔组织填充;局部可见变性坏死区,周围有新生的胶原纤维,直径大小不等;还可见代谢活跃的成纤维细胞及大量的肌纤维细胞;②实验组主韧带胶原原纤维直径大于对照组,经比较有显著性差异(t=10.23,P<0.05)。结论女性盆底器官脱垂患者存在胶原形态的异常,可能是导致女性盆底组织膨出的因素之一。Objective To observe ultrastructures of cardinal ligament by using transmission electron microscopy (TEM), and the collagen fibril diameters of cardinal ligaments was also measured to explore pathogenesis of pelvic organ prolapse (POP). Methods 10 biopsy specimens of cardinal ligament of women with POP confirmed and underwent total vaginal hysterectomy (TVH)and anterior and posterior colporrhaphy over a period from June 2006 to June 2007 in Department of Gynecology and Obstetrics, Longgang District Central Hospital of Shenzhen City, were enrolled as study group and another 10 biopsy specimens of cardinal ligament of women with benign uterine diseases underwent TVH in the same period were selected as control group..The specimens of cardinal ligaments were taken in the operation and uhrastructure of cardinal ligaments was studied by using TEM, and diameters of collagen fibril of cardinal ligaments were measured. Results①In the study group, arrangement of collagen bundles presented irregular and twistable shape. The broken structures could be seen and refilled by connective tissues. In the local areas, degeneration and necrosis could be seen, and in the surrounding, there were renewal collagens with varied of diameters, collagenoblasts and myofibroblasts with active metabolisms were also seen; ②The mean diameter of collagen fibril of these patients with POP were larger than that in the control group and the difference was significant (t = 10. 23 ,P 〈0.05). Conclusion The female patients with POP have morphological abnormalities in collagen fibers, which may be one of etiological factors to cause bulge of pelvic floor tissues.
