慈溪市参加新型农村合作医疗农民住院疾病构成及流向

摘　　要：目的了解慈溪市参加新型农村合作医疗农民住院疾病构成及选择不同医疗机构就医情况,为完善新型农村合作医疗制度提供依据。方法随机抽取慈溪市3个镇2006年度参加新型农村合作医疗农民的全部住院病例(4808例次),以国际疾病分类(ICD-10)为标准进行疾病分类,对住院疾病构成和流向进行分析。结果住院病人疾病排名在前5位的依次是消化系统疾病、肿瘤、循环系统疾病、呼吸系统疾病、损伤及中毒,占69.58%;在市内医院住院的农民占总住院例次的60.52%,市外医院住院的占39.48%。结论要加强慈溪市医疗卫生机构建设,提高对肿瘤、循环系统疾病和损伤等重点疾病的防治能力;通过调整起付线、完善补助比例、合理设置定点医疗机构、健全双向转诊制度等措施,引导农民合理选择医疗机构,更多地利用基层卫生资源和基本医疗服务。Objective To investigate the disease composition of inpatients and patients′ choice for the hospital by peasants in the new rural cooperative medical system,in order to provide basis for improvement of the new rural cooperative medical system.Methods All 4808 case times of inpatients in 2006 from 3 towns of Cixi city were taken at random.According to the standard of International Classification of Diseases-10th Revision （ICD-10）,the disease composition of inpatients and patients′ choice for the hospital by peasants were analyzed.Results Digestive system…… disease,tumor,circulatory system disease,respiratory system disease,injury and poisoning are the first five diseases,accounting for 69.58 % of all diseases.The peasants whose choice of hospitalization is in the city account for 60.52 % of all case times of hospitalization,while the peasants whose choice of hospitalization is out the city account for 39.48 % of all case times of hospitalization.Conclusion We should enhance the construction of health or ganization in Cixi city,increase the power for prevention and treatment of key diseases such as tumors,circulatory system diseases and injuries.We also should instruct the peasants to rationally choice hospital so as to use more primary medical resource and service by way of regulating the base line of medical reimbursement,consummating the reimbursable proportion,setting medical establishments more rationally and perfecting the two-way referral system.
