云南嗜人按蚊分布区疟疾防治对策的现场应用效果评价

摘　　要：目的评价"嗜人按蚊分布区疟疾综合防治对策"研究成果在疟疾回升或爆发点现场的应用效果,为该类疟区今后防治工作提供依据。方法用回顾性调查方法,对盐津、彝良、永善三县2003～2006年期间疟疾点状爆发和回升地区的现场防控资料,进行流行病学整理和分析。结果盐津、彝良和永善3县2005～2007年疟疾发病占全市11县的92.65%。发生于盐津县豆沙镇石门村和彝良县牛街镇花果村的疟疾点状爆发及大幅回升,属历史高疟区的当地病例传播引起;发生于彝良县钟鸣乡麻窝村和永善县码口乡烟坪村的疟疾病例,属于历史无疟区的输入性突发疫情。通过应用嗜人按蚊区疟疾发病率>10/万地区的防治对策,及时控制疟疾的爆发、回升及输入性疫情。各村点次年疟疾发病率下降55.56%～100%;输入性疟疾病例及时治愈,没有发生二代病例,次年亦未发生疫情。结论认真执行分类指导、分层防治疟疾的原则,就能有效的控制云南嗜人按蚊分布区的疟疾流行。Objective To evaluate the results in implementation of malaria control measures in areas with sporadic out-break and resurgence of malaria in areas with An. anthropophagus as the main transmission vector. Methods The data concerning malaria control in the couonties of Yanjin, Yiliang and Yongshan during the period of from 2003 to 2006 were epidemi-ologically analyzed. Results Malaria infections in the three counties occupied 92.65% of the total malaria cases occurred in the 11 counties of Zhaotong Prefecture City in 2005-2007 and there sporadic outbreak and resurgence of malaria infections occurred with a sharp increase due to local and imported malaria infections. Malaria incidence was reduced to 55.56%-100% in sporadic malaria oubreak areas after adopted integrated control measures. The imported malaria cases cured withour occurrence of secondary infections and there no malareia cases were detected in the second year after control. Conclusion The prevalence of malaria in yunan Province can be controlled by following the principles of guandance based on local types of transmision patterns and stratified control, in areas with An. anthropophagus as the main transmission vector.
