摘  要:目的研究在甲醛致痛大鼠模型中鞘内注射奈福泮对脊髓背角环氧化酶-2(COX-2)表达的影响,以探讨奈福泮镇痛的脊髓机制。方法雄性SD大鼠40只,随机分为对照组(C组)、模型组(F组)、生理盐水组(NS组)及盐酸奈福泮组(N组)。鞘内置管后5 d,F组、NS组及N组大鼠于左后足掌部皮下注射5%甲醛100μL,注射甲醛前20min,NS组鞘内注射20μL生理盐水,N组鞘内注射5μL盐酸奈福泮(100μg)后经导管注入15μL生理盐水冲管,C组足底注射100μL生理盐水。采用疼痛加权评分评价疼痛行为学,并在足底注射2h后将所有动物处死取脊髓膨大处,采用免疫组织化学法观察脊髓背角COX-2表达。结果与C组比较,F组、NS组脊髓背角COX-2表达增加(P<0.05);与F组、NS组比较,N组脊髓背角COX-2表达降低(P<0.05)。结论鞘内注射盐酸奈福泮可抑制甲醛炎性疼痛引起的脊髓中COX-2表达增加,可能与奈福泮的镇痛作用有关。Objective To investigate the effect of intrathecal (IT) nefoparn on the express)on of cyclooxygenase - 2 in dorsal horn of spinal cord in rat model with peripheral pain. Methods Thirty - two male SD rats were randomly divided into 4 groups ( n = 10 each) : Group Ⅰ (100uL normal saline was injected into the plantar surface of left hindpaw. ). Peripheral pain was induced by injection of 5 % formalin 100uL into the plantar surface of left hindpaw in Group Ⅱ ,group Ⅲ and group Ⅳ. In group Ⅲ , normal saline 20uL was injected intrathecally before interplanetary formalin injection. Nefoparn 5uL+ normal saline 15uL was injected in group Ⅳ. Pain behavior was assessed by PIS score. All the animals were killed at 2 h after interplanetary formalin (or NS) injection after assessment of pain behavior and cyclooxygenase- 2 expression was determined. Results The cyclooxygenase - 2 expression in group Ⅱand Ⅲ was significantly higher than that in group I but it was significantly lower in group Ⅳ (P 〈 0.05). Conclusion Intrathecal nefoparn could inhibit the cyclooxygenase - 2 expression in the spinal dorsal horn induced by interplanetary injection of formalin.
