长球囊血管成形术联合长支架置入术对糖尿病肢体动脉闭塞症患者血管内皮功能的影响

摘　　要：目的:比较长球囊血管成形术联合长支架置入术(LBPTALS))与普通球囊血管成形术联合普通支架置入术(OBPTAOS)对内皮分泌功能的影响。方法:选择经下肢血管造影证实为股浅动脉闭塞的糖尿病肢体动脉闭塞症患者43例,随机分为LBPTALS组和OBPTAOS组。术前、术后即刻、术后2h分别采下肢静脉血样,测定血清一氧化氮(NO)浓度和NO合酶(NOS)活力。结果:2组患者靶病变情况差异无统计学意义。LBP-TALS组的手术时间明显短于OBPTAOS组(P<0.05)。LBPTALS组术后即刻和术后2h的血清NO浓度、NOS活力高于OBPTAOS组(均P<0.05)。结论:LBPTALS对糖尿病肢体动脉闭塞症患者血清NO浓度、NOS活力的影响均小于OBPTAOS,这可能是前者通过影响内皮的分泌功能,最终减少再狭窄的机制之一。Objective：To investigate the effects of long balloon percutaneous transluminal angioplasty plus long stent implantation （LBPTALS） on secretion function of endothelium in patients with diabetes limbs artery occlusion. Method： A total of 43 patients with superficial femoris artery occlusion were randomly divided into two groups： long balloon percutaneous transluminal angioplasty plus long stents （LBPTALS） group and ordinary balloon percutaneous transluminal angioplasty plus ordinary stents （OBPTAOS） group. Limb vein blood was collected before angioplasty, immediately and 2 hours after stents implantation. Then the content of serum nitric oxide （NO） and activity of nitric oxide synthase （NOS） were measured. Result： There was no significant difference of the characteristics of the target lesions between the two groups. The during time of operation in LBPTALS group was less than that in OBPTAOS group （P〈0. 05）. The serum level of NO and activity of NOS were higher in LBPTALS group than those in OBPTAOS group immediately and 2 hours after angioplasty （P〈0. 05）. Condusion..Serum level of NO and activity of NOS were decreased after LBPTALS operation in patients with diabetes limbs artery occlusion. It might be the mechanism of less restenosis underlying they influence the secretion function of endothelium.
