摘 要:目的探讨应变成像(SI)检测冠心病局部心肌舒张功能异常的临床应用价值。方法应用SI技术获取53例冠心病患者和42例正常对照者左心室各壁不同节段的纵向应变曲线,测量快速充盈期、心房收缩期各节段应变的变化幅度ΔεE、ΔεA和ΔεE/ΔεA比值。对比分析冠心病组狭窄冠状动脉供血节段(狭窄节段)和对照组相应正常节段(正常节段)的各项参数。结果冠心病患者狭窄节段快速充盈期的应变上升幅度明显变小、消失,甚至倒置。冠心病组各室壁狭窄节段ΔεE测值较对照组相应节段明显减低,两者的ΔεA无明显差异,而两者的ΔεE/ΔεA比值差异有统计学意义。结论SI技术可无创、定量检测冠心病局部心肌舒张功能异常。Objective To explore the clinical value of strain imaging (SI) in quantitative detection of regional myocardial diastolic function in patients with coronary artery disease (CAD) . Methods Fifty-three patients with CAD and forty-two normal control subjects were enrolled in the study. Longitudinal strain curves in the basal, mid, and apical segments of LV walls were obtained. The increased amplitudes of strain value during rapid filling period (△εE) and atrial systolic phase (△εA)were measured, and △εE/△εA were calculated. According to the results of coronary angiography, the data in narrowed segments supplied by narrowed coronary arteries in CAD and in the corresponding normal segments in the controls were compared. Results The values of longitudinal strain in narrowed segments of CAD during rapid filling period were significantly smaller, disappeared, or even inversed. △εE in Narrowed segments were significantly lower than those in the normal segments, No significant differences were found in △εA, while statistical significance were found in △εE/△εA between narrowed and normal segments. Conclusions SI may De used as a new technique for detection of regional myocardial diastolic function noninvasively and quantitatively.
