彩色多普勒超声对椎-基底动脉供血不足患者治疗前后椎动脉血流的对比研究

摘　　要：目的探讨彩色多普勒血流显像(CDFI)在椎-基底动脉供血不足(VBI)的诊断和治疗中的价值。方法对50例临床拟诊为VBI的患者进行CDFI检查,分别测量治疗前后颅外段椎动脉的内径和血流动力学参数(收缩期峰值流速Vs、舒张末期流速Vd、阻力指数RI),正常对照组30例,并对结果进行统计学分析。结果治疗前椎动脉的内径小于对照组(P<0.05),且治疗前后内径无明显改变(P>0.05)。病例组治疗前椎动脉的Vs、Vd小于对照组(P<0.01),RI大于对照组(P<0.05);治疗后与治疗前相比,椎动脉的Vs、Vd增高(P<0.01),以RI增高为主要表现的患者的RI降低(P<0.01);病例组治疗后椎动脉的Vs、Vd与对照组接近(P>0.05),RI大于对照组(P<0.01);病例组中治疗前椎动脉的Vs、Vd、RI和内径均在正常范围者12例,其椎动脉的Vs、Vd与对照组接近(P>0.05),但用药后明显增高(P<0.01)。结论CDFI可为VBI的明确诊断和疗效评价提供客观依据。VBI患者椎动脉血流Vs、Vd降低、RI增高,临床治疗可以降低椎动脉RI、提高Vs、Vd。CDFI还可发现在超声诊断标准以外的VBI患者,并明确部分VBI的病因。Objective To probe the value of color Doppler flow imaging （CDFI） in the diagnosis and evaluation of vertebrobasilar insufficiency （VBI） treatment. Methods Fifty patients suspected as VBI underwent CDFI and thirty normal persons served as the control. The inner diameter and blood flow parameters （maximal systolic velocity Vs, minimal diastolic velocity Vd, resistant index RI） of extracranial vertebral arteries were measured before and after treatment, and statistics assessment was conducted. Results The inner diameters of the vertebral arteries before treatment were lower than those of the control group （P〈0.05）, and there was no statistics changes before and after treatment （P〈0.05） . The Vs and Vd P〈0.05 were lower, P〈0. 01 and RIs were higher, before treatment than those of the control group. The Vs and Vd after treatment were higher than those before treatment （P〈0.01）, RIs were lower in patients with higher RIs than that of the normal person （P〈0.01）; The Vs and Vd after treatment were close to the control （P〈0.05） and RIs were higher （P〈0. 01） than those of the control group. There were twelve patients with normal inner diameters, Vs, Vd and RIs before treatment and the Vs, Vd were close to the control group （P〈0.05） but higher than those in the control after treatment （P〈0.01） . Conclusions CDFI could provide positive evidence for diagnosis and treatment of VBI, that is, Vs, Vd are lower and RI is higher in VBI patients than the normal ; After treatment, the Vs, Vd may be raised and RI may be reduced. CDFI may provide some testimonies and etiology for VBI patients with normal blood flow parameters.
