摘 要:目的探讨超声对比增强诊断肝移植手术后并发症的临床价值。方法采用低机械指数实时超声造影方法,对29例原位肝移植手术后患者,经彩色多普勒查成像检查可疑发生血管并发症患者行超声对比增强检查,并与彩色多普勒血流成像、计算机断层血管重建、血管造影对照。结果(1)超声对比增强可清晰显示肝移植术后的大血管走形及通畅情况,与彩色多普勒血流成像比较超声对比增强大大降低肝动脉栓塞的假阳性;(2)超声对比增强可实时反映组织的微灌注状况,极具敏感性;(3)超声对比增强可以更清晰地显示无扩张的胆道,对于肝内脓肿与胆汁瘤的鉴别诊断优于普通超声。结论超声对比增强为超声波的一种新血流显像方式,在诊断肝移植手术后血管并发症的诊断中有重要的临床价值。Objective To estimate the clinical value of contrast enhanced ultrasound (CEUS) in diagnosis of vascular complications in the post-liver transplantation. Methods Twenty-nine patients with orthotopic liver transplantation were diagnosed as potential vascular complications using color Doppler flow imaging (CDFI), confirmed with real-time contrast enhanced ultrasonographic imaging under a low mechanical index. The results were compared with those of CDFI, computed tomographic arteriography (CTA), and digital subtraction angiography (DSA). Results 1. Compared with CDFI,CEUS provides an accurate assessment of patency and flow direction in the posttransplant patients, false positive rate of hepatic artery thrombosis was 0% compared with CDFI. 2. CEUS revealed a higher sensitivity in depicting and confirming the microvascular tissue perfusion. 3. CEUS improves the image quality of un-dilated bile duct, and differentiated biloma from abscess. Conclusions CEUS plays an important role in the diagnosis of vascular complications in the post -liver transplantation.
