腹腔镜超声在泌尿外科腹腔镜手术的临床价值

摘　　要：目的初步探讨腹腔镜超声检查术在泌尿外科腹腔镜手术的应用价值。方法18例泌尿外科腹腔镜手术术中应用专用腹腔镜超声探头,协助完成7例多囊肾手术并与单纯应用腹腔镜手术的患者对比;协助完成11例肾脏肿瘤腹腔镜下肿瘤切除术。结果对于多囊肾患者,腹腔镜超声能指导外科医师腹腔镜下有效清除深部囊肿,确认肾血管及扩展的集合系统,避免手术并发症;对于肾肿瘤患者,腹腔镜超声较常规经腹超声更清晰显示肿瘤边界及血供,从而指导外科医师既能完整切除肿瘤,避免肿瘤残留,又能最大限度保留正常肾组织。结论腹腔镜超声有助于术中指导手术步骤及提供准确信息,腹腔镜超声检查术在泌尿外科腹腔镜手术中有重要指导价值。Objective To evaluate the clinical value of laparoscopic ultrasonography （LUS） in urinary surgery. Methods Polycystic kidney in 7 cases and renal tumors in 11 cases were excised under the LUS guidance and the results were compared with the traditional simple laparoscopic surgery. Results With laparoscopic ultrasonography guidance, we could differ the cysts from dilated pelvis and renal calices, so the damage to the collecting system and renal blood vessels were avoided. Compared with trans-abdominal ultrasound, LUS showed tumors boundary and blood flow more clearly. Under LUS guidance, tumors were excised more completely, so that remained tumor tissues would be avoided and normal renal tissues would be protected. Conclusions LUS may provide more precise surgical informations and may play an important role guiding laparoscopic operation in urinary surgery.
