摘  要:目的:观察羟基磷灰石复合人工骨板整体修眼眶内下壁爆裂性骨折的效果,比较单片式或桥连双片式修复的差异。方法:2004-06/2007-10河北医科大学第三医院收治眼眶内下壁骨折患者34例,采用羟基磷灰石复合骨板整体修复眼眶内下壁爆裂性骨折,其中13例骨折范围大、眼眶壁损伤重者采用桥连双片式,其余21例采用单片式。对比观察手术前后眼球内陷程度及复视缓解情况。结果:34例患者术后随访>3个月。单片组眼球内陷矫正19例(90.5%),复视消失18例(85.7%);桥连双片组眼球内陷矫正9例(69.2%),复视消失10例(76.9%),组间比较差异无显著性意义(P>0.05)。除1例有感染外,未发生材料宿主反应及其他并发症。结论:羟基磷灰石复合骨板整体修复眼眶内下壁爆裂性骨折效果好,并发症少;选用单片式或桥连双片式修复方法可取得同样效果。用桥连双片式方法是修复超大范围眼眶内下壁爆裂性骨折的有效方法。AIM: To study the effect of hydroxyapatite/bone plate composites in the repairing for blow-out fracture of orbital medial and inferior walls, and compare the uniwafer and bridging biplate. METHODS: Thirty-four patients of the blow-out fracture of orbital medial and inferior walls, who were admitted in the Third Affiliated Hospital of Hebei Medical University, were analyzed retrospectively from June 2004 to October 2007. Among them, 21 cases adopted uniwafer composites for repairing, while other 13 severely injured used bridging biplate. They were perioperatively surveyed for the degree of enophthalmos and the status of diplopia. RESULTS: Totally 34 patients were followed up over 3 months. The rectification of enophthalmos was found in 19 cases (90.5%) of the uniwafer set and 9 cases (69.2%) of the bridging biplate set. The abolition of diplopia was found in 18 cases (85.7%) of the uniwafer set and in 10 cases (76.9%) of the bridging biplate set. There were no significant differences between the two sets (P 〉 0.05). Beside one case appeared infection, no host rejection or complications were found in other cases. CONCLUSION: Hydroxyapatite/bone plate composites can effectually repair the blow-out fracture of orbital medial and inferior walls. It has fine indication and few complications. Both uniwafer and bridging biplate gain uniform results. Furthermore the method of bridging biplate is effective to repair wide-ranging blow-out fracture of orbital medial and inferior walls.
