过敏性紫癜患者血清抗中性粒细胞胞浆抗体及抗血管内皮细胞抗体检测

摘　　要：目的探讨血清抗中性粒细胞胞浆抗体和抗血管内皮细胞抗体在过敏性紫癜发病中的意义及与临床表现的关系。方法应用间接免疫荧光方法(IIF),对42例过敏性紫癜患者血清进行了抗中性粒细胞胞浆抗体(ANCA)及抗血管内皮细胞抗体(AECA)检测,结果42例患者中ANCA的阳性率为4.76%,均为核周型(p-ANCA)。AECA的阳性率为71.4%。其中IgG-AECA阳性率为42.9%,IgM-AECA阳性率为28.6%。在伴有肾脏损害的患者中ANCA和AECA均阳性。结论通过对过敏性紫癜患者血清ANCA和AECA的检测,证实检测过敏性紫癜患者血清ANCA作为本病的辅助诊断指标意义不大,而对AECA的测定具有更重要的意义。但二者在肾脏损害方面均具有意义。Objective To study the roles of serum level of Anti - neutrophil cytoplasmic antibody（ANCA） and Anti - Endothelial Cells antibodles（AECA） in the pathogenesis and clinic.al manifestation of Anaphylactoid purpura. Methods ANCA and AECA of 42 patients of Anaphylactoid purpura were examined by indirect immuofluorescence（IIF）. Results The positivity rate of ANCA was 4.76% , and all ANCA - positive patients were with P - ANCA. The positivity rates of AECA, IgC - AECA and lgM - AECA in patients of Anaphylactoid purpura were 71.4% , 42.9% and 28.8% , respectively. Both ANCA and AECA were positive in the patients with renal complietion. Conclusions The detection of AECA level is usetul to identify Anaphylactoid purpura, but the ANCA is less valuable in the diagnosis of Anaphylactoid purpura. Both ANCA and AECA are valuable in the diagnosis of Anaphylactold purpura with renal complication.
