摘 要:目的探讨鼻息肉中嗜酸粒细胞趋化因子(Eotaxin)的表达与嗜酸粒细胞(Eos)活化的关系;糖皮质激素对其抑制作用。方法设置三组分别为鼻息肉组、鼻息肉地塞米松治疗组及正常鼻黏膜对照组;其中鼻息肉地塞米松治疗组入院后,术前接受地塞米松20mg,静脉滴注,qd×5。Pharma-ciaCAP系统测定样本血清中ECP的水平;HE染色、免疫组织化学技术观察Eos和Eotaxin在标本黏膜中的表达;分别行统计学分析。结果与地塞米松治疗组、正常对照组比较,鼻息肉组血清中ECP水平(16.47±5.38μg/L)及组织中Eotaxin(50.85±32.22)表达明显增强(P<0.05),Eos活化增多;ECP水平与Eotaxin表达相互间呈正相关性(r=0.647);糖皮质激素对鼻息肉血清中ECP水平(6.26±3.13μg/L)及组织中Eotaxin(15.21±11.32)表达有显著的抑制作用(P<0.05);正常对照组血清中ECP水平(3.57±1.49μg/L)及组织中Eotaxin(13.08±10.62)表达水平低;鼻息肉地塞米松治疗组及正常黏膜对照组之间差异无显著性(P>0.05)。结论鼻息肉血清中ECP水平升高,组织中Eotaxin表达增强,且相互之间呈正相关。糖皮质激素能抑制外周血中Eos、组织中Eotaxin表达和Eos活化。Objective To study the relationship between expression of eotaxin and eosinophils(Eos) activating in nose polyps(NP) ;study the essential function of corticosteroid in them and as one of main mark of activated eosinophils. Methods The specimens of twenty nasal polyps, twenty dexamethasone and twenty control subjects with similar sex and age were studied : three groups ( nose polps, dexamethasone, normal control). Twenty patients with NP were treated by DXM (20 mg, qd × 5 ) as dexamethasone group, the level of ECP in serum were detected by Pharmacia CAP system;and the pathologic samples of nose polps tissue were stained with HE;the expression of Eotaxin were observed by IHC (immunohistochemistry). The results were analysized by SPSS and linear correlation. Results Compared with the dexamethasone and normal contral groups, ECP level( 16. 47 ± 5.38 μg/L) and Eotaxin cells (50. 85 ± 32. 22) were expressed at high levels ( P 〈 0. 05 ) in nose polyp tissue, and the activated eosinophils increased. The expression of Eotaxin positively correlated with the level of ECP ( r = 0. 647 ) ; Glucocorticoids prominently inhibited ECP level(6. 26 ±3.13 μg/L) and the expression of Eotaxin cells( 15.21 ±11.32) (P 〈0. 05) ; ECP level(3.57 ± 1.49 μg/L) and Eotaxin cells( 13.08 ± 10. 62) were expressed at low levels in normal control group (P 〈 0. 05 );there wasn't significant statistical difference between the dexamethasone and normal contral groups(P 〉 0. 05). Conclusion ECP level and Eotaxin cells increase in nose polyps tissue. There is a significant correlation among them (r = 0. 647 ), while glucocorticoids could prominently down-regulated the expression of eotaxin and inhibited eosinophils activation.
