摘 要:按照2001年世界卫生组织(WHO)淋巴瘤最新分类标准,分析和总结新疆石河子地区125例B细胞性非霍奇金淋巴瘤(B-NHL)的临床病理组织学及免疫组化特点,以探讨其在B-NHL的诊断和鉴别诊断中的价值。依据2001年WHO淋巴瘤新分类标准,运用临床病理形态观察及免疫组化对125例B-NHL进行临床病理研究。125例B-NHL患者发病平均年龄为47.6岁(1.5岁~81岁),男女发病比例为2.4∶1。发生部位以淋巴结内为最常见(占60%),其次为胃肠道(占20%),还可见于涎腺等部位。按WHO分类,本组125例B-NHL确定了9个类型,未观察到淋巴结边缘区B细胞淋巴瘤、脾边缘区B细胞淋巴瘤、毛细胞白血病和B-前淋巴细胞白血病,其中最常见的3个类型依次是:弥漫大B细胞淋巴瘤(45例,36%),滤泡淋巴瘤(40例,32%),粘膜相关淋巴组织结外边缘区细胞淋巴瘤(22例,17.6%)。一些B-NHL的亚型有特异性免疫表型,对诊断和鉴别诊断有所帮助。病理形态是淋巴瘤诊断的基础,免疫表型在诊断和分型中起重要作用,二者与临床特征结合可使多数淋巴瘤得到明确诊断。To discuss the significance of diagnosis and differentiated diagnosis of B NHL with clinical pathology and immunohistochemistry feature by 125 cases B-cell non-Hodgkin's lymphomas(B-NHL) in Shihezi district of Xinjiang based on the new WHO classification of lymphoid neoplasms. 125 cases of B-NHL were consecutively selected from archives of the Department of pathology in Shihezi University School of Medicine. The clinicopathology and Immunohistochemistry of 125 cases B-NHL were studied based on the new WHO classification. The average age of 125 patients with B-NHL was 47.6 years(from 1.5 to 81 years), B-NHL were more common in males than females (2:4: 1). Most of the B-NHL (60%) arised in the lymph nodes, then gastrointestine (20%), salivary glands and so on. 125 cases could be further classified into 9 subtypes according to morphological and immnophenotypical feature: among the 125 cases lymphomas,diffuse large B cell lymphoma, follicular lymphoma and extranodal marginal zone lymphoma were the more common subtypes, accounting for 36% (45 cases) ,32% (40 cases) and 17.6% (22 cases) of all cases,respectively. Some of B-NHL subtypes have special Immunohistochemical that can help for diagnosis and differentiated diagnosis. Morphologic observation is basic mathod to classify B cell NHL, immunohistochemical study also plays an important role in further subtyping. A combination of both modalities are sufficient for arriving at an accurate diagnosis in most cases of B-NHL.
