40例川崎病患儿微小病毒B19感染的近期分析

摘　　要：目的进一步探讨微小病毒B19(HPVB19)与小儿川崎病(KD)发病的关系。方法应用巢式PCR法和ELISA法对40例川崎病(KD)患儿(观察组)及30例随机挑选本院门诊健康体检儿童(对照组)进行B19-DNA检测和B19 VP2 IgM检测。对观察组KD患儿中HPVB19-DNA检测阳性的与阴性的两组中应用心脏彩超检测冠状动脉内径值变化进行比较。结果观察组B19-DNA阳性检出率为27.5%(11/40),对照组B19-DNA检测均为阴性,两组比较有显著差别(P<0.01)。观察组B19 VP2 IgM阳性检出率为27.5%(11/40),对照组均阴性,两组比较有显著差别(P<0.01)。B19 DNA和B19 VP2 IgM一致率为100%。40例观察组中HPVB19-DNA检测阳性与阴性两组中,心脏彩超冠状动脉内径值变化有显著差异(P<0.01)。40例川崎病患儿在性别、年龄、常见临床表现、预后等方面无差别。结论HPVB19与小儿川崎病(KD)发病的有关,HPVB19可能是小儿川崎病(KD)病原之一;HPVB19与冠状动脉扩张或瘤形成可能相关,HPVB19感染所致冠状动脉扩张程度较重。Objective To investigate the relationship between human parvovirus B19 infection and Kawasaki disease in children Lately.Methods Forty children with KD serve as observing group, Thirty normal children serve as control group, using the nested-PCR detect microvirus B19-DNA in plasma and ELISA was used to detect serum B19 VP2 IgM. In the observing group,we compared the data of bore in coronary artery between theB19-DNA positive and theB19-DNA negtive group. Results Of the observing group, the positive rateforB19-DNAis 27.5 % （11/40）, of the control group, no one is positive. There are significant difference between the two groups（ P 〈 0.01 = . Of the observing group, the positive rate for B19 VP2 IgM is 27.5% （11/40）, of the control group, no one is positive.There are significant difference（ P 〈 0.01）. Crude agreement between the detecting results of B19 DNA and B19 VP2 IgM was100%. Of the observing group, the data of bore in coronary artery of the HPVB19-DNA positive group and negtive group, is significant（ P 〈 0.01 = . Conclusion The infection of HPVB19 may be an important factor for KD;The situation of coronary artery dilataltion in KD infected HPVB19 is more seriously, and HPVB19 may be the important factor of coronary artery dilataltion.
