不同分级的慢性移植物抗宿主病与B淋巴细胞相关性的临床研究

摘　　要：目的探讨不同分级慢性移植物抗宿主病(cGVHD)与B细胞分布特征关系,以期寻求其早期诊断及预后的实验室依据。方法回顾性研究2002年以来,资料完整的61例cGVHD患者的临床资料及免疫细胞流式细胞仪细胞表型结果;分析不同分级状态下,cGVHD患者B淋巴细胞比例的动态变化。cGVHD分级标准依据2005年,国际健康发展计划协会提出了cGVHD的分级评价标准。结果B淋巴细胞在不同分级cGVHD组间差异明显,造血重建组与不同分级状态下cGVHD组间有统计学意义,造血重建初期、无cGVHD发生、轻度、中度和重度时分别为(2.14±0.97)%、(8.49±2.25)%、(12.08±3.32)%、(13.45±2.64)%、(16.14±5.88)%,P=0.002。结论不同分级cGVHD状态下,B淋巴细胞表现出不同的分布趋势;通过对不同分级cGVHD患者B淋巴细胞亚群的分析,为B细胞在cGVHD发生机制中的作用提供了证据。并可能通过临床监测B淋巴细胞群变化为临床判定cGVHD分级及其预后提供实验依据。Objective To investigate the relationship between different status patients with chronic graft-versus- host disease （cGVHD） and the distribution of B lymphocyte of patients with allogeneic hematopoietic stem cell transplantation（allo-HSCT）. To find some experiment evidences for the early diagnosis and treatment for cGVHD. Methods B lymphocyte subsets in the peripheral blood were detected by monoclone antibody and flow cytometry in 61 patients with different status of cGVHD after allo-HSCT. The principle for the status of cGVHD was based on the National Institutes of Health Consensus Development Project on Criteria for Clinical Trials in cGVHD in 2005. Results The proportions of B lymphocyte were different within the different status of cGVHD. They were significantly different within the proportions at the early stage of hematopoiesis, non-cGVHD, mild cGVHD, moderate cGVHD and severe cGVHD （2.14±0.97）%,（8. 49±2.25）%,（12.08±3.32）%,（13.45±2.64）%,（16.14±5.88） %,respectively（ P = 0. 002）. Conclusion The percents of B lymphocyte subsets are different within the different status cGVHD. With more serious cGVHD, the proportions of B cell are significantly higher. It may help for early diagnoses and treatment of cGVHD.
