嵌顿性腹股沟斜疝的无张力疝修补术临床研究

摘　　要：目的:探讨无张力疝修补术是否适用于嵌顿性(伴有全腹膜炎)腹股沟斜疝。方法:根据疝嵌顿后腹膜炎体征情况,统一手术切口、术式及抗生素使用剂量应用天数,对36例腹股沟嵌顿性斜疝病人与同期108例择期的腹股沟疝病人分为两组对比。采用mesh prefix plug定型产品作无张力疝修补术。比较两组患者手术时间、术后并发症、切口感染率、复发率。结果:与对照组(n=108)相比,研究组(n=36)手术时间明显延长,差异有显著性(t检验,P<0.01)。术后并发症、切口感染率、复发率差异无显著性(Fisher确切概率法,P=0.085,P=0.378,P=0.561)。结论:无张力疝修补术同样适用于伴有全腹膜炎的嵌顿性腹股沟斜疝。Objective：To investigate whether the tension-free hernia repair was suitable for incarcerated indirect inguinal hernia with panperitonitis.Methods： Thirty-slx patients with incarcerated indirect inguinal hernia and concurrent 108 patients with inguinal hernia who were registered for operation were studied prospectively.The material used for tension-free hernia repair was perfix plug of Bard surgical product company.The operative time ,postoperative complication,incisional infection and recurrence rate were compared between two groups.Results ：The operative time was significantly longer in incarcerated group than in control group （t test P=4.96〈0.01）, while the postoperative complication ,incisional infection and recurrence rate had no significant difference between them（Fisher：P=O.085 ,P=0.378, P=0.561）.Conclusion：The tension-free hernia repair is similarly true for incarcerated indirect inguinal hernia with panperitonitis.
