摘 要:目的通过研究细胞增殖核抗原(PCNA)和细胞凋亡在前列腺上皮内瘤(PIN)组织中的表达,探讨细胞增殖和凋亡在前列腺上皮内瘤形成过程中的作用。方法采用免疫组织化学方法和脱氧核糖转移酶介导dUTP缺口末端标记(TUNEL)技术检测12例前列腺上皮内瘤和12例良性前列腺增生(BPH)组织中细胞增殖核抗原和细胞凋亡的表达。结果PIN中PCNA阳性细胞主要分布于腺上皮的基底细胞层与分泌细胞层,在BPH中主要见于腺上皮基底细胞层。PIN和BPH中细胞增殖指数分别为(15.92±4.40)%和(8.33±2.93)%,两组间比较有显著性差异(P<0.01)。在BPH和PIN组织中凋亡细胞主要分布于腺上皮的基底层,分泌层内少见。PIN和BPH中细胞凋亡指数分别为(8.33±2.99)‰和(4.00±1.85)‰,两组间比较有显著性差异(P<0.01)。结论PIN组织中细胞增殖和凋亡活性显著高于BPH,增强的细胞增殖和凋亡活性参与了前列腺上皮的恶性转化,在前列腺上皮内瘤的发生、发展中起重要作用。Objective To study the role of cell proliferation and cell apoptosis in the genesis and development of PIN, the alteration of cell proliferation and apoptosis states are investigated in BPH and PIN, respectively. Methods There were a total of 24 prostatectomy specimens. 12 cases in PIN and 12 cases in BPH, respectively. Immunohistochemical staining was used to detect the activation of PCNA. TUNEL histochemistry was used to detect apoptosis. Results PCNA positive cells and apoptotic bodies were located mainly in the basal and secretory cell layer of PIN, whereas in the basal cell layer of BPH. Moreover, their number decreased from the basal layer to the luminal layer of gland epithelial. The cell proliferation index were (15.92±4.40)% in PIN and (8.33±2.93)% in BPH, respectively. The cell apoptosis index were (8.33±2.99)%v in PIN and (4.00±1.85)‰ in BPH, respectively. Both the cell proliferation index and the cell apoptosis index in PIN were significantly higher than in BPH (P〈0.05). Conclusion The elevated activation of cell proliferation and cell apoptosis are involved in the malignant transformation of prostate epithelium. The disruption of balance between cell proliferation and cell apoptosis make up the cell kinetics basis of the genesis and development of PIN.
