摘　　要：目的了解男男性接触者（MSM）首次同性性行为的发生情况及影响因素。方法2006—11／2007—02，在北京市调查了541名男男性接触者，内容包括社会人口学特征、首次性交对象、性取向和首次同性性行为发生情况等。结果调查541名男男性接触者，其中首次性行为为同性的发生率为：4．57／100人年（95％CI=4．19～4．96），首次同性性行为发生的年龄中位数为21．0岁（95％CI=21．0～22．0）。在多因素Cox比例风险模型中，出生年份晚（1970—1980年HR=2．68；95％CI=1．83～3．94。〉1980年HR=6．17；95％CI=4．11～9．28）、少数民族（HR=1．44；95％CI=1．06～1．96）、首次性行为对象为同性（HR=2．18；95％CI=1．76～2．68）、双方自愿（HR=0．65；95％CI=0．50～0．85）与首次同性性行为发生的关系有统计学意义。结论少数民族、首次性行为对象为同性和双方自愿对男男性接触者首次同性性行为的发生有影响，男男性接触者中青少年是重点干预对象。Objective To investigate the factors associated with first homosexual intercourse among men who have sex with men（MSM）. Methods From November, 2006 to February, 2007, 541 MSM were investigated in Beijing. The questionnaire included birth date, ethnicity, education level, registered permanent residence, the object who was the first sexual intercourse with, sexual orientation and how the first homosexual intercourse happened. Results Among 541 MSM, the average incidence rate from birth to first homosexual intercourse was 4.57 per person-years （95 % CI = 4. 19 - 4.96 ） with median age 21.0 years （ 95 % CI = 21.0 - 22.0）. In the multivariate Cox proportion hazard regression model, late birth date（ 1970 -1980 HR, 2. 68; 95% CI =1.83 -3.94. 〉1980 HR=6. 17; 95% CI =4. 11 -9.28）, minority（HR, 1.44; 95% CI =1.06 - 1.96）, first sex with men （ HR, 2. 18 ; 95% CI = 1.76 - 2.68 ）, voluntary initial sexual with men （ HR, 0. 65; 95% CI = 0. 50 -0. 85 ） were the factors independently associated with the first homosexual intercourse. Conclusion Minority, first sex with men and voluntary initial sexual with men increased incidence rate from birth to initial homosexual intercourse men among MSM, and HIV intervention should be paid more attention to adolescent MSM.
