彩色多普勒超声评价肝静脉血流在肝移植术后的临床价值

摘　　要：目的应用彩色多普勒超声（CDFI）检查肝移植术后（OLT）移植肝形态及血流动力学的变化，重点探讨肝静脉血流频谱形态在肝移植术后的应用价值。方法19例原位肝移植患者，术后观察肝脏大小，内部回声，以及肝内外胆管、肝动脉、门静脉、肝静脉二维图像，血流参数，重点观察肝中静脉血流频谱形态变化特征。结果肝移植术后无并发症及有并发症治愈的患者，其肝中静脉频谱由HV2型或HV1型转为HV1或HV0型；有并发症未治愈的患者，其肝中静脉频谱持续呈HV2改变。结论肝静脉血流频谱形态变化特征能提示肝移植术后并发症的存在与否，并能观察其疗效及判断其转归，具有重要的临床应用价值。Objective To evaluate the clinical value of the spectrume changes of hepatic vein with color Doppler ultrasound to exame transplantation liver shape and homodynamic changes after orthotopic liver transplantation. Methods Liver size internale echo, hepatic arteria, vena portae and the spectrume changes of hepatic vein were obsvered with color Doppler ultrasound in 19 patients after orthotopic liver transplantation. Results The hepatic vein spectrume turn HV2 or HV1 to HV1 or HV0 in non complications and complications were cured after orthotopic liver transplantation;the hepatic vein spectrume sustained HV2 in patients of complications were not cured. Conclusion The spectrume changes of hepatic vein can be as of the important index that judge cueative effects and results after orthotopic liver transplantation , its have important clinical application value.
