益气活血汤对慢性肺心病急性加重期患者血内皮素-1和一氧化氮水平影响的研究

摘　　要：目的:探讨益气活血汤对慢性肺心病急性加重期患者血内皮素-1(ET-1)和一氧化氮(NO)水平的影响。方法:将80例慢性肺心病急性加重期患者随机分为治疗组和对照组,各40例。2组均给予抗感染、解痉平喘、吸氧等常规治疗;治疗组在常规治疗基础上加用益气活血汤口服。疗程均为14天。结果:治疗组显效率为62.5%,总有效率为97.5%;对照组显效率为32.5%,总有效率为80.0%,2组显效率、总有效率比较,差异均有显著性意义(P<0.05)。2组经治疗后ET-1含量均下降(P<0.05)、NO含量均升高(P<0.05):治疗组2项指标改善较对照组更明显(P<0.05)。治疗组在改善血液流变学指标、升高动脉血氧分压、降低二氧化碳分压方面均优于对照组(P<0.05)。结论:益气活血汤对慢性肺心病急性加重期具有较好的临床疗效,其机制可能通过降低血中ET-1、提高NO含量,从而降低肺动脉压,达到治疗的目的。Objective:To observe effect of Yiqi Huoxue Decoction(YHD)on serum levels of endothelin-1(ET-1)and nitric oxide(NO)in patients with chronic pul- monary heart disease(CPHD)acute exacerbation.Methods:Eighty patients were equal- ly randomized into two groups.The two groups received routine treatment for anti-inflam- mation,relieving spasm and asthan,and oxygen inhalation,and the treatment group re- ceived oral use of YHD additionally.The treatment lasted 14 days.Results:The markedly effective rate was 62.5% and the total effective rate was 97.5% in the treat- ment group,and was 32.5% and 80.0% in the control group respectively,the differ- ence being significant(P<0.05).Serum ET-1 level decreased and NO content in- creased in the two groups after treatment(P<0.05 compared with those before treat- ment),and the changes of the ET-1 and NO were obvious in the treatment group(P<0.01 compared with the control group).The effect on improving hemorheological index- es,increasing partial pressure of oxygen in artery,and decreasing partial pressure of car- bon dioxide in the treatment group was superior to the controI group(P<0.05).Con- clusion:YHD has certain effect for chronic pulmonary heart disease acute exacerbation and the therapeutic mechanism may be related with the decrease of pulmonary artery pres- sure by decreasing serum ET-1 and increasing NO.
