将AIDS／STD预防服务纳入城市社区卫生服务中的有效性及可行性探讨

摘　　要：目的探讨在城市社区卫生服务中心诊疗工作中,纳入艾滋病/性传播疾病(AIDS/STD)预防服务的有效性及可行性。方法在北京市海淀区的4家社区卫生服务中心,由经过培训的社区医生面向就诊的娱乐场所女服务员开展AIDS/STD预防服务。通过对社区医生访谈和目标人群问卷调查的结果,评价其有效性及可行性。结果2007年4月30日至7月30日,共调查歌厅、发廊等女性服务人员296人。通过预防服务活动,目标人群性病知识总知晓率由18.6%提高到60.9%(χ2=107.38,P<0.01);艾滋病知识总知晓率由17.6%提高到57.6%(χ2=92.28,P<0.01);85.9%的目标人群表示愿意接受医生给予的AIDS/STD知识。参与活动的社区医生,AIDS/STD知识有了系统的提高,对目标人群有了更好的了解,表示愿意将这项工作进行下去。结论将AIDS/STD预防服务融入社区卫生服务诊疗工作中,能有效地提高目标人群的AIDS/STD知识,得到了目标人群和社区医生的认可,具有一定的推广意义。Objective To evaluate applicability and effectiveness of integrating AIDS/STD prevention into routine medical care service at urban community health centers. Methods A pilot AIDS/STD prevention program was undertaken at 4 community health centers in Haidian district of Beijing, where appropriate preventive service was provided by trained general practitioners （GPs）to female workers in entertainment venues （FWIEVs）. The effectiveness and applicability of this program were assessed by interviews with GPs and questionnaires filled in by the target population. Results Follow- ing prevention activities, the integrated knowledge score regarding STDs among FWIEVs increased from 18.6% to 60.9% （x^2 = 107.38,P 〈 0.01）, while the integrated knowledge score of HIV/AIDS increased from 17.6% to 57.6% （x^2 = 92.28,P 〈 0.01）. Of the surveyd FWIEVs,85.9% felt satisfied with these actlvltles. All the GPs involved have improved their knowledge of AIDS/STD and relationship with FWIEVs, and stated that they would be interested in continuing such service. Conclusion This program has successfully integrated AIDS/STD preventive services into the routine health practice of the community health centers, effectively improving the knowledge of AIDS/STD and has been accepted by both FWIEVs and GPs,and so it may be further expanded.
