摘  要:为探讨血清神经元特异性烯醇酶(NSE)、血浆内皮素-1(ET-1)与新生儿缺氧缺血性脑病(HIE)的关系,分别用酶联免疫法和放射免疫法测定25例正常足月新生儿和59例足月HIE新生儿血清中NSE浓度和血浆ET-1浓度,并观察其动态变化(生后24小时内、第3天、第7天)。结果HIE中、重度组生后3天内NSE显著高于对照组和轻度组(P<0.01);第7天逐渐下降,但重度组仍高于轻、中度组和对照组(P<0.01)。ET-1浓度于生后3天内HIE组显著高于对照组,且重度组高于中度组,中度组高于轻度组(P均<0.01);第7天逐渐下降,但中、重度组仍高于对照组和轻度组(P<0.01)。24小时内和第3天NSE与ET-1浓度呈正相关(P<0.01)。NSE、ET-1浓度在生后3天内升高和在第七天持续不降者预后不良。提示NSE与ET-1浓度改变可作为新生儿HIE脑损伤的早期生化指标,为早期干预提供机会和理论依据。This paper discusses whether neuron specific enolase(NSE) concentration in serum and endothelin-1 (ET-1) in plasma are correlated with the neurologic condition of full term neonatal hypoxic-ischemic encephalopathy (HIE). 59 HIE infants (20 cases of light, 20 of middle and 19 cases of severe degree)and 25 normal infants' serum NSE concentrations were measured by enzyme immunoassay dynamically(within 24 hours,at the 3rd day and the 7th day of life); ET-1 in plasma were measured by radioimmunoassay. NSE and ET-1 within 3 days of life were related to the degree of HIE. Infants with HIE had higher concentrations of NSE and ET-1 than those of the normal infants. Neonates with severe and middle degree had higher NSE and ET-1 than those with light degree and the normal. At the 7th day,NSE and ET-1 decreased,the severe degree,s NSE were still higher than the other's ;ET-1 of infants with severe and middle degree were higher than the light degree and normal infants. There was a positive correlation between NSE and ET-1 within 3 days after labor. Those whose concentrations of NSE and ET-1 increased within 3 days of life and didn,t decrease at the 7th day had adverse outcome. The results indicate that NSE and ET-1 are reliable biochemical markers for early estimates of hypoxic-ischemic brain damage in full term HIE infants
